Certificate of Acceptance
System Type:
BCM 400, 90 T7316e telephones, With Call Pilot Voice Mail 80 seats.
Customer hereby acknowledges that the telecommunications system
including all equipment, licensed software and accompanying services (the
"System") sold by AT&T Indiana to Customer pursuant to Contract
Number 16151219 has been installed, and that the System is properly
functioning. Customer hereby accepts the System and acknowledges that
the equipment Warranty period is now in effect from 8/24/06
to 8/23/07
Customer acknowledges that it has 30 days after installation completion to
test the System and deliver to AT&T t~is signed Certificate of Acceptance or,
alternatively, a written notification clea;rly describing a specific System
element's failure to meet identified specifications. If Customer does not
return the Certificate of Acceptance or a written notice of nonconformity
within such time period, the System will be deemed accepted on the 10th
business day after installation completion.
