Consulting Services Agreement
This is an agreement made as of this ~'>'~ day of Q~&.~,,I,,~

between The City of Jeffersonville, Indiana and L&W Research, a market research
firm.
Client (City of Jeffersonville) hereby retains L&W Research (Consultant) to perform
services as described in Attachment A. Consultant agrees to perform the services in
consideration of the compensation described in Attachment B.
This Agreement consists of this document together with Attachment A - Project
Requirements and Work to be performed, and Attachment B - Fee Schedule. This
agreement between the City and Consultant supercedes all prior written and oral
understandings. This agreement may only be amended, supplemented, modified or
canceled by a duly executed written instrument.
In executing this agreement, the undersigned also acknowledged their authority to bind
the parties to all terms and conditions.
In witness whereof, the parties hereto have made and executed this agreement as of the
day and year first written.

Attachment A
Project Requirements
1. Description of Services
The primary purpose of this project is to assist the City of Jeffersonville, Indiana, in
conducting the 2006 Jeffersonville Community Survey.
2. Scope of Services
Client (City of Jeffersonville, Indiana) shall provide addresses, print and prepare
surveys and mail (or otherwise distribute) surveys to selected participants.
Consultant (L & W Research) shall:
a. Attend meetings related to the survey design and implementation
b. Assist with survey revisions
c. Collect and tabulate survey responses
d. Conduct statistical analysis of survey responses
e. Prepare and deliver a written report of the data analysis
f. Present survey results
3. Project Schedule
The time frame for the completion of the Consultants' activities is four (4) weeks
from receipt of the completed surveys.
Modification
City, without invalidating the Conlxact, may make changes to the service as
necessary. If the change requires an adjustment in the Contract Price, the Consultant
shall transmit to the City the costs of such changes by letter. Please note that no
changes to the contract shall be allowed unless a written change order is agreed upon
and executed by both the City and the Consultant.

Item 1
Consulting fees for this item will be billed on a time and materials basis at $100.00 per
hour and reimbursable expenses with a budget not to exceed $7,150.00.
