HomeMy WebLinkAbout2005 Laser Surgery AgreementLASER SURGERY DISCOUNT AGREEMENT--2005 The John-Kenyon Eye Center is in agreement with the City of Jeffersonville for the following LASIK Discount Program: For a one-year period in effect starting January 1, 2005 to December 31, 2005 the John-Kenyon Eye Center will provide IntraLASIK m'~d Advanced Surface Ablation (A.S.A) with custom technology (CustomVue or Wavelight) at a 20% discounted price. This 20% discount is subject to future fee increase. II. All employees and elected officials of the City of Jeffersonville will be eligible for this program after 12 months of employment with the city. City employee or elected official must verify that they have had at least 12 months employment wi~la the city. Contracted employees are not eligible for 20% discount or co- payment options. a. The co-payment agreement is as follows: IntraLASIK or A.S.A with Custom Technology Regular Price $2265 per eye 20% discount $1812 per ey.9 Employee Payment (30%) City Payment (70%) First Eye: $543.60 $1268.40 Second Eye: $543.60 $1268.40 $1087.20 $2536.80 b. All spouses of City of Jeffersonville employees will receive IntraLASIK at a 20% discounted fee. The City of Jeffersonville will not be responsible for any co-payment. This fee includes initial consultation, 12 months of postoperative care at the John-Kenyon Eye Center and a Two Year Enhancement Policy*. The initial consultation and laser vision correction procedure will be pertbrmed at the Louisville Clinic located at 4040 Dntcl'anans Lane. III. The City of Jeffersonville will allow the John-Kenyon Eye Center to make presentations and display information regarding this discount program. The John-Kenyon Eye Center will provide promotional materials for this purpose. IV. The City of Jeffersonville will not participate in any other discounted refr~r,g~ogram.
