LASER SURGERY DISCOUNT AGREEMENT
The John-Kenyon Eye Center is in agreement with the City of Jeffersonvitte for the following LASIK Discount
Program:

For a one-year period in effect starting January 1, 2004 to December 31, 2004 the John-Kenyon Eye
Center will provide IntraLASIK and Advanced Surface Ablation (A.S.A) at a 20% discounted price. This
20% discgun~ is subject to. -~ture fee increase.
All employees mad elected officials of the City of Jeffersonvi!le will be eligible for this program after 12
months of employment with the city. City employee or elected official must verify that they have had at
least 12 months employment with the city. Contracted employees are not eligible for 20% discount or co-
payment options.
a. The co-payment agreement is as follows:
IntraLAS1K or A.S.A
Regular Price $1975 per eye
20% discoum $1580 per eye
Employee Paymem City Pavmem
First Eye: $316 $!264
Second Eye: $316 $1264

b. All spouses of City of Jeffersonvilte employees will receive IntraLASIK at a 20% discounted fee. The
City of Jeffersonvilte will not be responsible for any co-payment.
This fee includes initial Consultation, ! 2 months of postoperative care a:t the JohmKenyon Eye Center
and a Lifetirile'Enha~ement Poiic¥*. T,,e m.Um c,,n~At,,:,,.n and laser ~,o..m c.~..ec~,o,, ~,o ..... e
will be p~rformed at the Louisville Clinic Iocated at ~040 Dutchrnans Lane.
III. The City of Jeffersonville will allow the John-Kenyon Eye Center to make presentations and display
information regarding this discount program. The John-Kenyon Eye Center will provide promotional
materials for this purpose.
IV. The City of Jeffersonville will not participat~ in any other disc0unted re~ra~program.
