LAS1K SURGERY DISCOUNT AGREEMENT
The John-Kenyon Eye Center is in agreement with the City of Jeffersouviile for the
following LASIK Discount Program:
I. F;r a three year period the John-Kenyon Eye Center will provide LASIK at
a 15°A discounted price. This 15% discounted price is subject to future fee
increase.
A. All employees and elected officials of the City of Jeffersonville will
receive the following, after 12 months of employment with the city.
The co-payment agreement is as follows:

B. AIl spouses of the City of Jeffersonville employees will receive LASIK'at
the 15% discounted fee of $1678.75 per eye. The City of Jeffersonville
will not be responsible for any co-payment.
C. This fee includes the initial coasuitation, 12 months ofpostoperativeeare
at the John-Kenyon Eye Center and *a Lifetime Enhancements Policy.
The initial consultation will be done in the Jeffersonville office with the
LASIK procedure being performed at 4040 Dutchmans Lane in
Louisville.

The Cit~ of Jeffersonville will allow the John-Kenyon Eye Center to make
presentations and display information regarding this discount program. The John-
Kenyon Eye Center will provide promotional materials for this purpose.
