We have completed the annual review of your group program with Companion Life Insurance Company.
We are pleased to inform you that no rate increase is needed at this time. Your current rates, which have
been renewed for the upcoming policy year, are as follows:

AD&D $.05 Per $1,000
Dental
Single $6.00
Employee + 1 $6.00
Family $6.00
These rates will be reflected on your February 2003 Group Billing Notice.
Your agent has been notified and will be happy to answer any questions you may have.
Some items to keep in mind: in order to be insured under the group insurance policy your eligible
employees must be actively at work full-time. This means that they must be working at least 30 hours
per week or the number of hours defined in your contract that a full time employee must work. Should an
employee's employment terminate or should an employee's work hours be reduced below this minimum
the employee's coverage must terminate. Please refer to your group insurance policy.
