NOTICE OF PENDING INSURANCE
EXPIRATION

Maxus Lease Nos.: 30361 and 30363
Ladies and Gentlemen::
We noticed that the insurance policy for the above referenced leases will be expiring on September 1,2003. Per our
Lease Agreement, you are required to provide evidence of both Property and Liability Insurance for this equipment.
Please request that your insurance provider provide to us immediately for each Lease indicating the
following:
1. An ACORD 25 form (CERTIFICATE OF LIABILITY INSURANCE) with the following information:
(a) Reference should be made to Maxus Lease Nos. 30361 and 30363.
(b) The Additional Insured and Loss Payee should be listed as: MAXUS LEASING GROUP,/NC.,
31300 Bainbridge Road, Cleveland, Ohio 44139.
(C) Such Policy may not be cancelled or altered without at least thidy (30) days prior written notice to
Maxus Leasing Group, Inc.
2. An ACORD 27 form (EVIDENCE OF PROPERTY INSURANCE) with the following information:
(a) Reference should be made to Maxus Lease Nos. 30361 and 30363.
(b) The Additional Insured and Loss Payee should be listed as: MAXUS LEASING GROUP, INC.,
31300 Bainbridge Road, Cleveland, Ohio 44139.
(c) Such Policy may not be cancelled or altered without at Feast thirty (30) days prior written notice to
Maxus Leasing Group, Inc.
Within fifteen (15) days of this Notice, please fax these Certifications to 440.519.2401 or mail to:
