Parker jr. called the meeting ~o order at 9:39 A.M., and on call of the roll those members present were Mayor Raymond J. Parker Jr., Sewage Collecti°n Office D%rector Jim Matra, and Administrative Assistant Donna Baird filled in 'for Personnel and Safety Director Harold Lakeman in his absence due tO illnesS. Chief Eider expi~ined'~he~e' has beeh several accidents there, and there also was a stop sign at Graham Street. Mayor Parker said we need to take this under advisement and come up with another plan. PAGE 2 OCTOBER 9, 1995 SEWER TAP IN BEST WAY TRUCKING Attorney Ronald Fifer aPPeared before the Board representing Dave Sumitt, Best Way Trucking, 4403 Hamburg Pike ~o request permission to tap in on City sewer lines. Attorney Fifer said Best Way is building a new building on his property and needs to hook onto the City sewer lines. Attorney Gillenwater said he needs to sign a wavier to give up his right to remonstrate against annexation if he has not signed one before. Director Matra said there will be a S25.00 investigation fee. Director Matra made the motion to approve the sewer tap in for Best Way Trucking at the stipulations mentioned by the City Attorney and a 925.00 investigation fee, seconded by Member Baird, and carried unanimously. SOLICITOR'S PERMIT Nancy voyles of Henryville is requesting a permit to go door to door in Jeffersonville selling for Mid Western Marketing, Inc. Mayor Parker explained that Ms. Voyles will have to go through the Police Department for and investigation and give names and social security numbers of all employees, there will be a S25.00 fee. Chief Elder will bring his recommendation back to the Board next week. SEWER PROBLEMS ON PARK PLACE Joe Reschar is appearing before the Board for his mother Mrs. Reschar, 403 Gilmore, at the corner of Park Place. For 19 years her basement has flooded everytime it rains. A backflow valve was i~nstalled, the floor drains are plugged, there is so much pressure it is blowing thr6~gh the concrete and it still backs up in her basement. 'Mr. Reschar explained that Mrs. Res~a? is 82 years old and she is carrying 2 gallon buckets of Water ~t of her basement everytime it rains. Mr. Rescha~ i~ reqUesting a gate ValVe to be put in her sewer. He checked her clean out this morning and there is still 2 foot of water in it. Mayor Parker agrees ~lth Joe its been hastled with long enough. Director Coleman said we can do this, Mrs. Reschar has to sign a notification saying they can do the work and they will not be responsible for anyother damage 'that might occur, if we damage anything while we are there, we wi~!l be responsible. Director Coleman recommends putting the valve in for Mrsl Reschar. Director Matra made the motion to approve the gate val~e ~t 403 Gilmore DriVe, s~conded by Member Baird, and carried ~i~animousty. UTILITIES REGULATION Attorney Gillenwater would like the authority from the Board to get some help from another law firm who is more familiar with Utilities RegulatiOn. Attorney Gillenwater iS %hinking of Barnes and Thornburg, Jim Guttings firm to take a lobk 'at some of our Ordinances to maybe modify them, it might alleviate some of the odor problems we've had. There being no further business to come before the Board of Public Works and Safety, Director Lakeman made the motion to adjourn at 10:10a.m., seconded by Director Matra, and carried unanimously. Center Director Consultative Optometw The following is an addendum to the agreement of reduced fee for service for Radial Keratotomy between the City of Jeffersonville and the John-Kenyon Eye Center signed April 4, 1994. The original agreement was to mn for a period of three years. TNs addendum fails under the timeframe governed by the original agreement and will expire April 4, 1997. Under this addendum agreement: In addition to offering Radial Kemtotomy, the John-Kenyon Eye Center is offering Lamellar Keratectomy iLK) as part of the same reduced fee for service agreement. Police, Fire, and Public Safety employees would pay an initial examination fee of $60. If it is determined the iLK) procedure is needed, the City of Jeffersonville would pay $1,232 toward the total cost of LK. o The totai cost per. eye would be $1,500.00 or $3,000 for both eyes. With the $1,232 paid by the City of Jeffersonville, the total cost for both eyes to be paid by Police, Fire, and Public Safety employees would be $1,768. o The governmental agency discounted price of $1,500.00 per eye would also be extended to ail other employees of the City of Jeffersonville and their respective spouses. The City of Jeffersonville would not be responsible for any co-payment with regard to "other" employees or their respective spouses. This addendum also extends the offer of a discounted price for Radiai Keratotomy (P.K) of $800 per eye to the spouses of employees of the City of Jeffersonville. However, the City of Jeffersonville would not be responsible for any co-payment with regard to this group of individuals.
