This partnership program exists to promote employment of Indiana University Southeast students in this geographic area with the goal of retaining them as residents of the state of Indiana upon graduation from IU Southeast. Company: The City of Jeffersonville Internship Student: Jordan Blanton Position Title: Planning Intern Beginning Date: September 6. 2005 Ending Date: December 9 2005 (14 weeks of employment) Hourly Rate: $8.29 Hours per Week: 20 Total Dollars Earned: $2321.10 Total FICA @ (7.65%~: $ 177.57 Subtotal: $2498.67 Total dollar amount to be paid to the student intern: $2498.67 The student intern should be paid on a weekly or biweekly basis. The student will be considered an employee of The City of Jeffersonville and be subject to all of the policies of The City of Jeffersonville. Indiana University Southeast will forward a check sent by an anonymous donor to The City of Jeffersonville shortly after the signing and approval of these forms. The agency will supervise the student's internship and will require that time sheets be kept for all hours worked by the student. The student, the agency, and Career Services will each receive a copy of the time sheets on a weekly or biweekly basis. For the agency tc receive funds at the beginning of the internship period, the agency must accept the above agreement and must provide the university with an invoice on agency letterhead for the amount of funds as stipulated in the agreement.
