It is the policy of Pool Man, Inc. to have a valid, current credit card on file for any service to be scheduled.
The service fee is to be prepaid prior to any service being performed for any customer choosing not to have a credit card on file.
No service will be scheduled for any customer with an outstanding balance. Customers with past due amounts/outstanding balances will not be
permitted to schedule service until all outstanding balances are paid in full. ***
PAYMENT TERMS(SELECT ONE):
-/ Maintenance agreement: We will bill the above address monthly for the weekly fountain service(@$50 a week)plus chemicals.
• Adjust balancing chemicals such as pH,alkalinity,sanitizer,etc.(One Time Pay-Extra Charge for:algaecides,clarifiers,stain products,
flocculants,etc.).This does not include Natures and/or Spa/Pool Frog Products.Monthly Debit pays for ALL chemicals used.
It is the homeowner's responsibility to shock the pool as needed including after heavy bathing loads and after heavy rains.
• Backwash/clean filter and check equipment for proper operation
• If any problems are noted,you will be notified with an estimate and repair time will be scheduled.
• For hot tub service we will drain and refill your hot tub every 4 months only.If your hot tub needs drained and refilled more frequently there will be
extra charge based on how often you prefer
The following terms and conditions apply to the service:
1. Safety Issues:The homeowner/resident is responsible to maintain code compliance with the city on issues such as fencing and gates,and all other
safety issues.
2. Water level:We will add water while on site if needed and will turn it off when we leave.It is the homeowner's responsibility to maintain proper
water level.
3. Chemicals:All chemicals are stored on our vehicles.This includes sanitizers,balancing chemicals,problem chemicals,etc.
4. Rain Visits:In the event of rain,we will perform a chemical and equipment check only.No refunds will be given for this type of visit.In the event
of heavy rain or storms we will perform your service on the following day.
5. Inclement weather:If an additional run is requested because of bad weather there will be an additional charge of$100 per hr($25 per'''A hr)
6. Termination:Service can be terminated with a 48-hour notice.Please call the store to cancel your service.
7. Pets:The homeowner/resident is responsible to contain and restrain their pets.
8. Additional cleanup chart_es:If the pool is unusually dirty at time of service or in the event that the pool receives excessive debris and/or dirt load
due to landscaping,poor drainage,or storms there will be an additional charge of$100 per hour($25 per each'A hour)for each hour over the normal
maintenance service time.
9. Customer Satisfaction:Our goal is to obtain and maintain your complete satisfaction.If a job is not satisfactorily completed,it is the customer's
responsibility to notify The Pool Man within 7 days(preferably sooner)so that we can look into the matter and resolve it quickly.If you do not bring
the issue to our attention during this time,we will not be able to issue any credits.
10. Repairs:We arc happy to perform all repairs and most remodeling for your pool.Call us for a bid.Our labor charge to dispatch a technician to your
pool is$100 per hour.The$100 service run fee is waived for all current weekly service customers.
The above prices and specifications are satisfactory and arc hereby accepted.I hereby authorize you to perform the above-mentioned services and agree to pay
according to the terms outlined' this agreement.
