WHEREAS, Humana as the third party administrator of the
City's self-funded health insurance program requires that a
Letter of Credit in the amount of $10,000.00 be posted which
may be drawn upon in the event that City funds are insufficient
to meet the charges as processed by Humana in said capacity;
and,
WHEREAS, the

THEREFORE, BE IT RESOLVED by the Common Council of
City of jeffersonville that it hereby authorizes the Mayor
of the City of jeffersonville to execute the application for
irrevocable standby letter of Credit attached hereto on behalf
of the City of jeffersonville in order to fulfill the
contractual requirements by which Humana serves as the City's
third party administrator for its self-funded health insurance
program, said application is attached hereto.

RESOLUTION
BE IT RESOLVED that on the 27th day of September, 1991,
Ronald L. Vaughn filed a petition with the jeffersonville Plan
Commission, Jeffersonville, Indiana, requesting a
recommendation concerning

thence Northwardly and parallel with Pfeiffer's line a

set for hearing and notice by publication as required by law,
having been heretofore given, and after hearing evidence, the
jeffersonville Plan Commission hereby makes a recommendation of
denying petition to the Common council that the above
designated real estate be changed from R-2, Multi-Family
Residential, to I-l, Light Industry, and that the Secretary
certify a copy of this Resolution and deliver same to the
clerk-Treasurer of the City of jeffersonville, Indiana, and
further that the Common Council of the City of jeffersonville
is hereby requested to give their due consideration to said
