City of Jeffersonville is responsible for providing a flush vac truck in order to
clean all pipe segments prior to grout installation.
Televac will mobilize to jobsite&setup signs/cones/traffic barricades as
needed.Apply AV-100 Chemical Grout Mix to pipe joints as needed in various
pipe segments throughout the city of Jeffersonville,IN. Install(2)6"x24"CIPP
Pipe Patches @ Iat04185.Install(1) 18"x48"CIPP Pipe Patch @ san15208.

1. Proposal is valid for 30 days from the above date. Hours are estimated based on the information provided. The final invoice will
be based on actual time and material incurred using the above rates.
2.Payment Upon Project or Work Completion. Net 30 days. (5% processing fee for credit card payments)
3. Work must be given to Tele-Vac Environmental in adequate time to allow for completion by our team. Tele-Vac Environmental
shall not be held liable for liquidated damages or subject to any other penalties for work issued without adequate time to complete.
4. Existing pipe conditions that cause equipment to become stuck will be the responsibility of the owner. Tele-Vac Environmental
will not be held liable for the retrieval, excavation, or repair of existing pipe or line installed by others.
5. Any unauthorized additions, deletions or alterations to any part of this document will null and void these prices and this proposal.
