Contract for Performance
Contract for performance is made this 15"' day of March ,2019 between the
Undersigned Purchaser(herein after called"Purchaser")and The Original Group,LLC. DBA The Monarchs.
1. Purchaser:
SARA SIMPSON
Organization: CITY OF JEFFERSONVILLE PARKS DEPT
2. Place of Performance: RIVERSTAGE
Performance Date: FRIDAY, 6-28-19 Hours: 8:30 TILL 10:30
The Monarchs require a minimum of 15 minutes break time per hour of performance
4. Type of Performance: Concert
5. Contract Price: $2000.00 Deposit: None
6. Provisions for Payment: (check) XX (cash) (other)
Checks should be made payable to"THE MONARCHS"and presented to the leader or the leader's agent
"PRIOR"to performance. The Monarchs reserve the right to delay performance until payment is made.
7. Purchaser to provide:
Access to stage area 3 hours prior to performance
Reserved parking for band and crew near stage
Purchaser to provide front of house sound and lights
8. The recording,reproduction or transmission of The Monarchs performance is prohibited absent of written consent
from The Monarchs authorized agent.
9. Purchaser acknowledges and confirms that they have read and approve the terms and conditions set forth in this
contract and contract rider if applicable.
10. There is no cancellation privileges set forth in this contract unless specifically included in special provisions.
11. Any controversy of claim arising out of or related to this contract or the breach thereof shall be settled by
arbitration in accordance with the rules of the American Arbitration Association.
12. This contract must be returned within 15 days or The Monarchs reserve the right to cancel.
