AGREEMENT made (3/20/19) by and between named ARTIST (Tony& The Tan Lines) and
named PURCHASER(Jeffersonville Aquatic Center). The Parties agree to the following terms.
1. ARTIST - Tony&the Tan Lines
2. PURCHASER-Jeffersonville Aquatic Center
3. Date Purchased - Saturday, June 22, 2019
4. Place of Performance - Jeffersonville Aquatic Center
5. Performance time - 1:OOpm-4:OOpm
6. Audience/Guest Arrival Time -N/A
7. Agreement Price - $3800
8. Deposit-A$500 non-refundable deposit is required at signing of contract to hold the date.
9. Balance is due on(6/22/19) (cash or check made to Tony and the Tan Lines).
Check may be sent to 532 South 4th Street, Unit 201, Louisville,KY 40202
10. It is understood that PURCHASER shall have control of the services rendered hereunder,
subject to the terms and conditions in this AGREEMENT. This Agreement cannot be modified
or
amended except by in writing and signed by both parties. The validity, construction,performance
and breach of this Agreement shall be governed by the laws of the State of Kentucky.
11. This agreement is for the purchase of(performance date) on Artist's calendar. Note "date
change or exchange"is not permitted. "Date Change" shall be considered termination of this
contract. Terminating action by(Purchaser) of this Purchase Agreement by
Purchaser within 90 days of(performance date) shall be considered a breach of contract and
ARIST shall be entitled to full contract costs.
12. Purchaser shall provide power, a dressing area(if requested by Artist) and a stage area for
Artist's performance. Artist's equipment set-up and performance takes place in ONE
performance site only per details of this agreement and date.
13. If Artist is to perform outdoors, a tent covering entire stage area must be provided. A stage or
concrete platform(no grass, gravel, uneven ground, etc.)must be provided by Purchaser no
smaller than 20 ft. x 16 ft. unless discussed with Artist. Power from 3 separate circuits must also
be provided.
14. Artist's timely arrival and set-up at performance site shall be considered within Artist's
control. Artist's physical ability to perform and the functionality of Artist owned equipment shall
be considered within ARIST's control. In the event that Artist's performance is delayed to
prevented by any occurrence beyond Artist's control (such as inclement weather,power outage,
venue becomes uninhabitable, etc.), Purchaser shall remain liable for the full agreement price of
(Agreement Price) . If Artist must cancel for any reason,Artist will make all necessary
arrangements for acceptable replacement entertainment. If any member cannot make date of
performance due to illness, unforeseen events, etc, Artist shall provide acceptable replacement.

Performance Agreement
This contract is being entered into by and between Jeffersonville Parks Department and
Harvey L. Banister,Jr., dba Soul Kitchen,for the following service(s):
Date: TBD
Performace Venue: Jeffersonville Aquatic Center
Description: Pool Party
Fees and Compensation: $1,000
Services: Performers will be given access to the performance area not less than 2 hours
prior to event start time. Performers will not be required to perform beyond scheduled
time or have our fee reduced fee should the performance begin late due to circumstances
beyond our control.
Rain Policy: In the event of an unfavorable weather forecast,no payment is expected if the
event is cancelled before the band has begun setting up equipment. If rain or other
inclement weather should occur after the band has begun setting up, 10% of our fee will be
due. If rain or other inclment weather should occur after the performance begins,the band
will have fulfilled it's contractual obligation and the full fee will be due.

PERFOMANCE DETAILS:
3 Hour set with breaks.
Artist will provide their own sound.
COMPENSATION:
$1,000 to be paid the day of the performance.
*Artist can be paid in cash or check made out to Canaan Cox
*Artist will keep 100% of merchandise sales.
*Artist will receive 100% of payment rain or shine.
TRANSPORTATION/HOTEL:
Artist will provide.
ARTIST REQUESTS:
Venue will provide artist(s) with food and drink.
