Performance Contract
For a total fee of$1250 total including performance and travel fee:
-on October 29,2018.
-at Address: Big 4 Station Park -Jeffersonville,IN
-3 performer(s)and safety will arrive around 5pm and we will perform around 6pm. We will then check in with you--
can we text your cell to be discrete?--survey the dance area and begin fueling our tools and setting up.
-We will perform for approximately 2 hours each including a 10-15 minute break per hour.
1 LED Stilt Juggler+2 Fire Dancers and safety for 2 hours.
-We will use fire and/or LED Stilt Juggling if the weather is permitting.

-Please provide parking in front for us(please confirm you can do this)so we can easily transport the sound system and
gear.Will that be possible?

If you agree to these terms and want to confirm booking,please sign,date and make a$625 non refundable deposit(Included
in balance)made out to Matt O'Daniel or Art after Dark Entertainment. Deposit is non refundable unless we are unable to
perform due to illness or cancellation on our part.If the event is canceled for any reason Art after Dark Entertainment is not
responsible for returning the deposit nor under any obligation to perform for the rescheduled date,if there is one. If there is
inclement weather not allowing performance and Art after Dark Entertainment is on site then full payment is expected.
The full balance of$1250 is due in cash or cashier's check/money order within 2 weeks after the performance date of
October 29,2018,aka.November 12,2018 directly to us please at 3813 Sunrise Way Louisville,KY 40220.
