COMMERCIAL PEST CONTROL CONTRACT
THIS AGREEMENT, made and entered into between the undersigned BLACK DIAMOND TERMITE &
PEST CONTROL, hereinafter to be known as BLACK DIAMOND and Jeffersonville Fire Dept,
hereinafter to be known as Customer.
BLACK DIAMOND agrees to perform pest control services for control of common insects and rodents, a list
of which may be made available upon request
in the following areas: Interior located at 735 Wall St Jeffersonville, IN 47130
2006 Allison LN Jeffersonville, IN 47130; 1603 Truckers Blvd Jeffersonville, IN 47130; 2204 10" St
Jeffersonville, IN 47130; 5311 Highway 62 Jeffersonville, IN 47130
billing address if different: 2204 E 10t' St Jeffersonville, IN 47130
for a period of one (1)year beginning May 2017 with automatic renewal unless notified of cancellation.
Cancellation by either party must be made by thirty (30) day written notice after the period set forth above. If
Customer cancels contract before the completion of the initial period, the remainder of the contract shall be due
at the time of cancellation.
BLACK DIAMOND is to make one (1) call(s) per month and more after if necessary to control infestation. In
consideration of this contract, the customer agrees to pay BLACK DIAMOND $271.00 per month. If an initial
clean out service was not performed at the start of the pest control service, and one is deemed necessary, there
will be an additional charge. We offer a full warranty in between scheduled visits for the covered pests.
BLACK DIAMOND carries Workman's Compensation Insurance and General Liability Insurance in
accordance to State Requirements, certificates of which shall be provided upon request.
BLACK DIAMOND fu er guarantees that all work will be performed by trained and insured service
technicians. All pesticid shall bepproved by the Environmental Protection Agency and will be applied in
strict compliance 'tTfrally ap oved labels, state and local regulations, and the highest safety standards.

Payment according to terms.Charges are subject to 2%interest per month,collection fees,and reasonable attorney's fees.Block Diamond is not responsible for damage to
personal property that might occur due to our performing actions essential to job requirements as homeowner is responsible to have all areas prepared prior to our arrival
on site
