Contract for performance is made this 10th day of January ,2017 between the
Undersigned Purchaser(herein after called"Purchaser")and The Original Group,LLC.DBA The Monarchs.

Organization: Jeffersonville Dept. of Parks &c Recreation
2. Place of Performance: Jeffersonville River stage
3. Performance Date: Friday 7-7-17 Hours: 8:30 till 10:30
The Monarchs require a minimum of 15 minutes break time per hour of performance
4. Type of Performance: Conceit
5. Contract Price: $2000.00 Deposit. None

Checks should be made payable to"THE MONARCHS"and presented to the leader or the leader's agent
"PRIOR"to performance. The Monarchs reserve the right to delay performance until payment is made.
7. Purchaser to provide:
• House sound and lights provided by purchaser

8. The recording,reproduction or transmission of The Monarchs perforinance.is prohibited absent of written consent
from The Monarchs authorized agent.
9. Purchaser acknowledges and confirms that they have read and approve the terms and conditions set forth in this
contract and contract rider if applicable.
10. There is no cancellation privileges set forth in this contract unless specifically included in special provisions.
11. Any controversy of claim arising out of or related to this contract or the breach thereof shall be settled by
arbitration in accordance with the rules of the American Arbitration Association.
1.2. This contract must be returned within 1 S days or The Monarchs reserve the right to cancel.
