HomeMy WebLinkAboutSupplemental Agreement #7 HNTB SUPPLEMENTAL AGREEMENT, NUMBER SEVEN This Supplemental Agreement, Number Seven, to the AGREEMENT FOR PROFESSIONAL SERVICES, dated December 23, 2009 (the Agreement), between City of Jeffersonville (Owner) and HNTB Corporation (HNTB) is made effective as of date of Owner signature below. 1. HNTB shall perform and has performed the following services: See Attachment A — Scope of Services 2. HNTB shall perform the Services and deliver the related Documents (if any) according to the following schedule: See Attachment B — Schedule 3. In return for the performance of the foregoing obligations, Owner shall pay to HNTB according to the following terms: See Attachment C - Compensation Except to the extent modified herein, all terms and conditions of the Agreement shall continue in full force and effect. Wenzel, AICP, LEEDO AP Title. il el) Title: Vice President Date: / - Y 3- /3 Date: /1 Prepared by: HNTB Corporation City of Jeffersonville January 15, 2013 Supplemental Agreement, Number Seven Page 1 of 5 ATTACHMENT A SCOPE OF SERVICES FLOODWALL IMPROVEMENTS FOR BIG FOUR BRIDGE RAMP APPROACH At the request of the Owner on January 8, 2013, HNTB was directed to add a storage vault building to the design documents and contract plans, to include the additional post- design services for the storage vault building, and to update the Operations and Maintenance (O &M) Manual for Jeffersonville Flood Control and the US Army Corps of Engineers. Supplemental Agreement Number 4 and 6 are revised to include this Supplement's scope of work as follows: Task 1 - Develop Final Tracings (100 %) Drawings and Submittal HNTB will prepare final structural storage vault design, drawings, special provisions, and quantities to include in the construction cost estimate. The design will include the following components: • Foundation, floor, walls, roof and connections. HNTB will also update the Operations and Maintenance (O &M) Manual for Flood Control and the Corps. Deliverable for Task 1: • HNTB to electronically submit the drawings and other items to INDOT via INDOT's Electronic Records Managements System (ERMS) • HNTB to provide one (1) electronic PDF set of drawings, USP's, quantities, and cost estimate to the Owner • HNTB to provide one (1) electronic PDF set of drawings and calculations to the Corps • HNTB to provide one (1) electronic PDF of the O &M Manual Further assumptions include: • Scope will be limited to those tasks as shown in this supplement. • The storage vault building, in its entirety, will be comprised of cast -in- place, reinforced concrete, similar to most of the other existing storage vaults in the City. • The vault will be connected to the existing floodwall stem. • The plan drawings, in addition to what the design covers, will include details for a single steel door, passive ventilation through the door and in one side wall, two scuppers for roof drainage, roof waterproofing, roof flashing, and modifying the existing alley roadway around the building. • The floor will be sloped to drain toward the door with no other form of drainage inside the building. • Updates to the O &M Manual will only be for this particular closure structure. Amendments will be made to Table 2 in Section VIII and "Order of Erection" diagrams will be added to the end of the manual for this Mulberry Street closure structure. The diagrams will include the assembly details already developed as part of the contract plans. • Post - design services will be identical to those items covered under Task 5 in Supplement No. 4. Prepared by City of Jeffersonville HNTB Corporation Supplemental Agreement, Number Seven January 15, 2013 Page 4 of 5 ATTACHMENT C COMPENSATION FLOODWALL IMPROVEMENTS FOR BIG FOUR BRIDGE RAMP APPROACH Section 1 - Basis of Compensation a. The compensation to be paid for performance of the Services identified in Attachment A — Scope of Service, will be a lump sum amount of $28,000, which includes expenses, increasing the total compensation under the Supplemental Agreement Number 6 to the amount of $131,800. Notice to proceed for this work is provided once this agreement is signed. Invoices for the previous month's work shall be submitted monthly based on the percentage of completion of the work authorized in this amendment as of the date of each such invoice. HNTB may alter the compensation distribution between individual phases, tasks or work assignments to be consistent with the Services actually rendered, within the total lump sum amount.
