Letter of Agreement
November 27, 2012
This letter constitutes an agreement between The City of Jeffersonville, IN (Client) and University Research
Consultants (URC) for completing an Income Survey (project proposal attached hereto and incorporated herein by
reference). Project is to be completed according to the schedule outlined in the proposal.
In return for the services defined in said proposal, the Client agrees to pay URC $2,300 + the cost of door -to -door
and /or mailing. Payment will be made upon completion of the project and submission of the final report to the
client.
All checks should be made payable to University Research Consultants and mailed to the address as indicated
below.
To indicate acceptance of the terms of this agreement and the terms as outlined in the contract proposal, please
sign and return one (1) copy of the original proposal and one (1) copy of the letter of agreement to:
