As per attached documentation:
1. Add one (1) 15" Headwall (a) $1,008.00 /EA = $ 1,008.00
2. Add three (3) 18" Headwall a $1,060.00 /EA = $ 3,180.00
2. Regrade Ditch East of Station 38 +21 $3,520.00 /LS = $ 3,520.00
3. Credit for sod (aa $5,000.00 /LS = $ (5,000.00)
4. Delete 12' of 18" HP PP ® $118.80/LS = $ (118.80)
2. Delete Dud Headwall a $1,745.00/LS = $ (1,745.00)
3. Delete INDOT Stone 1473 (aa $1,435.46/LS = $ (1,435.46)
4. Add 4 Bar Saeens at Headwalls a $560.00 /EA = $ 2,240.00
ADD = $ 1,648.74
The method of payment for this change or extra work shall be:
An additional amount of one thousand six hundred and forty -eight dollars and seventy -four cents = $1,648.74

the following work.
ADDITIONAL WORK
Install four (4) safety gates for end of pipe. 1 V2" pipe stock 8" on center, anchor and pad lock
down to concrete at headwall with hinges on top.
