CLIENT requests and authorizes CH2M HILL, INC. (hereinafter "CH2M HILL") to perform the following Services:

Provide additional technical review, editing, document preparation, and analysis services to prepare the full semi-annual
consent decree report. The additional sections include reporting on the status of Jeffersonville's combined sewer overflow
operational plan, maximization of flow, the long-term control plan, and supplemental environmental projects.

Compensation will be paid on an hourly basis per the Standard Hourly Rates Schedule table included in Attachment B to the
Original Agreement with anot-to-exceed price of $6,500, including labor and expenses.
The Period of Performance for this Amendment is June 22. 2010 through July 31.2010
No other changes.
Services covered by this AGREEMENT will be performed in accordance with the Provisions and any attachments or
schedules. This AGREEMENT supersedes all prior agreements and understandings and may only be changed by written
amendment executed by both parties.

Proposal: Geoprobe® rock soundings and soil sampling for sewer extension
Stinger Lane Extension, Jeffersonville, Clark Co., IN
Members of the Board:
Thank you for allowing Geo Logic, Inc. to provide you with our services at the above referenced
facility. The scope of work includes advancing rock soundings to bedrock refusal up to a maximum
depth of 20' bgs at intervals of approximately 100'. In addition, we understand that one rock
sounding is proposed at each sewer manhole. We estimate that we can complete this project in
three (3) days of soundings. We understand that the borings will be staked or painted on asphalt
prior to our arrival. Our submittal package will include depths to rock or absence of rock beyond the
required depth of the sewer invert, and will reference the numbers staked by Blankenbeker and Sons.
Our proposal is detailed below:
TASK COST
Equipment mobilization, Geoprobe® track rig/crew, local, lump sum $150 $ 150.00
Rock soundings, sampling 1 of 10 refusals to describe bedrock, 3 days @ $1,200/day $ 3,600.00
Data preparation, spread sheet with rock depths/absences relative to boring location $ 200.00
Backfill holes w/ bentonite chips topped with asphalt cold patch, Estimate 50 @ $10/hole ~ 500.00
TOTAL ESTIMATED INVOICE AMOUNT ~ 4 450 00

wage and/or certified payroll, we will not exceed the above detailed $4,450 for the work detailed in

Re: Herb Lewis Road Sanitary Sewer Survey and Engineering design
Dear Board members:
The firms of Dixon Engineering and Blankenbeker and Son Land Surveyors, Inc., P.C., are jointly offering to
provide the necessary surveying field work and engineering design for this project. The plans and survey work
were originally done for Gilmore Construction, Inc. Gilmore construction has agreed to donate these plans and
the associated easements to the City of Jeffersonville. This proposal is to update these plans to meet City
specifications for bidding, include updating plans to include two additional sanitary sewer lines, one to Herb
Lewis Road and the other to the frontage road along 10`" street. Also included are updating the current permit
for the City. Rock soundings shall also be obtained by a reputable geotechnical firm, with those costs being
excluded from this agreement, and the results of these sounding will be added to the plans. Staking for the rock
soundings are included, but all other construction staking is excluded.
The fee for the above is $11,750.00. These fees shall be invoiced upon completion of the work and payment
made by the City within 30 days of receipt of the invoice. Additional work beyond the scope of this agreement
shall be invoiced at the following rates: Field surveying will be invoiced at $150 per hour. Senior Engineer shall
be invoiced at $125 per hour. Professional Land Surveyor shall be invoiced at $100 per hour. CARD manager
shall be invoiced at $75 per hour.
Work can begin work within one week of your notification to proceed. If this is acceptable to you, please sign
and return a copy of this letter.

Acceptance of Proposal: I/We agree that the above specifications and conditions are satisfactory and are hereby accepted.
Dixon Engineering and Blankenbeker & Son are authorized to do the work specified. It is further agreed that any out of pocket costs
such as copying fees, overnight fees, recording fees, application fees, etc. shall be added to the amount quoted. Payment will be
made upon completion of work. If the entire balance is not paid when due, I/V1/e agree to pay a late charge of 1.50% (18% APR) or a
minimum of $5.00 per month, whichever ' greater. In the ca of default of payment as agreed, I/We promise to pay any collection
costs including, but not limited to, fi ' f sand/or to ich are incurred to collect on this account.
