Ex. MH Sta. 16+21.26 Line "E" to MH 40 Sta. 0+74.0.? Line "f' had been installed previously.
2. Contract Item No.5 - Manhole, Type "C" - MH 40 Station 0+74.00 Line "f' deleted because MH had - $1,702.00
been installed previously on another contract.
3. Contract Item No. 7 - INDOT Surface Asphalt - 2.0 tons added to extend the driveway on Line "r' + $187.80
at approx. Sta. 1+50 to pave over storm sewer patching ,area. .
4. Contract Item No.8 - Surface Milling - Item decreased because the only milling performed was in front of - $4,800.00
either concrete or asphalt driveways. '
5. Contract Item No. 10 - Asphalt Price Adjustment - Deduction made beCaUse ofthe asphalt index prices. - $6,488.00
