Dear Mr. Elliott:
Recently I had the opportunity to meet with Department personnel to discuss the
operations for the City of Jeffersonville. One of the main objectives of the Risk
Control Department is to assist Alliance participants in reducing any existing and/or
potential liability exposures, which may be present in their daily operations.
Upon review of the information that I received during my assessment, I found no
areas Of major concern that would require immediate attention. I aro how~eve[
making the following recommendation§.
All pieces of park playground equipment should be installed over shock
absorbing surface material. Please see the enclosed information for types of
acceptable shock-absorbing material and installation specifications. (Applies
to several park playgrounds)
Tot type swing seats and regular swing seats should not be mixed in the
same swing bays. Tot swings usually require another individual to be located
nearby which increases possible collision hazards. (Applies to several park
playgrounds)
· There appears to be an old concrete equipment base still in the ground at
Optimist Park that could present tripping hazards. It should be removed.
The freestanding slide at Optimist Park has openings in the ladder that is
greater then 3.5 inches but less than 9 inches. This can present entrapment
hazards to small children. These openings can be closed by installing a
shield on the backside of the ladder to reduce hazards.
As you have questions regarding areas of liability exposure, or if we can be of
service in other ways, please call the Risk Control Department. We look forward to
