Tri County Animal Control to provide all A) Labor B) Supervision, C) Equipment D) Materials and Supplies E) Vehicles Animal Shelter 1) NO ANIMAL SHELTER PROVIDED Insurance Coverage Requirement Tri County Animal Control should provide the following Insurance requirements. 1) General Liability Policy adding City of Jeffersonville Government as Additional Insured A) General Liability Occurrence Limit of$1,000,000.00. B) General Liability Aggregate Limit of$2,000,000.00. 2) Workers Compensation Insurance_ • CONTRACT FOR PROFESSIONAL SERVICES This is a contract entered into by Tri County Animal Control, LLC an Indiana entity and the City of Jeffersonville, an Indiana municipal corporation on this 15th day of May 2025. The Provider's place of business is 3005 Industrial Parkway Jeffersonville Indiana 47130 and the Client's place of business is Quartermaster Court Jeffersonville Indiana 47130. SCOPE OF SERVICES 1) Emergency Animal Control will be available from 5:30 p_m_until 07:00 a_m_ Monday through Thursday and Friday 5:30 p.m. until Monday 7:00 a.m.for the following guidelines and Tri County Animal Control will respond to all calls for Emergency Animal Control Services to where a A. Dangerous or aggressive animal that is or may have the potential to become a threat to either person or other domesticated animals. B. Animals in life-threatening conditions including but not limited to Animal in high traffic highway Animal struck by motor vehicle. Animals at large experiencing a medical emergency. Potential to become harmed by a person or another animal. Animal that is trapped to where it is unable to free itself C. Police Arrest with Animal in Persons possession D_ Police/Fire/EMS needing assistance where Animal needs removed or contained_ E. Motor Vehicle Accident where Animal is involved. F. House Fire or Emergency Scene where Animal is involved. 4 Payment 1) The term of this contract shall be from January 1,2024 until December 31,2025. '2) City of Jeffersonville agrees to pay Tri County Animal Control the sum of$27,523.92 a year. 3) This contract should be governed by the laws of the City of Jeffersonville, County of Clark, in the state of Indiana and any applicable Federal,State,and local laws, rules, regulations,and ordinances. 4) Any changes to this agreement must be made in writing and agreed to by both parties. 5) Either party can terminate this agreement upon (120)days written notice to the other party. 6) Tri County Animal Control shall not incur or charge the client any other fees or expenses without prior written authorization of the City of Jeffersonville 7) Performance beyond the limitations set forth in this agreement shall be at the sole risk and responsibility of City of Jeffersonville and Tri County Animal Control shall not be obligated to pay for services exceeding the funding of this agreement. 8) Tri County Animal Control to, at its own-will be responsible for donations and to apply for and to receive grant funding for Tri County Animal Control In witness oft e •: -ement to e terms above,the parties or their authorized agents hereby affix their signatur:..
