The Agreement will begin in October 2024 under the terms and conditions Below. The agreed upon pricing is noted next to each description. Terms and Agreement 1. JOB DESCRIPTION FOR QUARTERLY MAINTENANCE Station#1 735 Wall Street Jeffersonville,IN 47130 Every 4 months-Clean Main line under men's sink through cleanout-$300 Station#2 2204 10th Street,Jeffersonville,IN 47130 Every 3 Months-Jet kitchen sink line and treat lines-$250 Station#3 1601 Truckers Blvd,Jeffersonville, IN 47130 Every 4 months-Jet kitchen sink line-$250 Every 4 months-Jet urinal-$250 Station#4 5311 E. 10th Street,Jeffersonville, IN 47130 Every 4 months-clean kitchen sink line-$250 Station #5 2006Allison Lane, Jeffersonville, IN 47130 Every 4 months- clean line under men's lavatory through cleanout-$350 Every 4 months-clean kitchen sink drain through cleanout-$350 2. Term This agreement will begin October 2024 and continue through October 2025. At that time the parties will reexamine the agreement to see if any changes want to be made. 3. Rights and Obligations of the parties -The customer shall now pay additional labor or service fees for the periodic maintenance of the above work described. However, in the case of part replacement/change,the cost of these parts shall be paid by the customer. -Damanges that may arise as a result of external factors such as fire, flood, and third parties intervention in the described work areas are not subject to this agreement. -Shears Drain Cleaning will prepare an invoice containing a detailed record of the operations performed during each quarterly maintenance and give a copy to the customer. 4. Miscellaneous - This agreement and maintenance schedules constitute the complete understanding of the parties to this agreement regarding the subject matter contained in this agreement and supersedes all other agreements and arrangements either oral or in writing. - This agreement and all the rights of the parties here under shall be governed by, construed and enforced with the laws of the state of Indiana. - In the event either part wishes to terminate this agreement,they will need to notify the other part within 30days of the next scheduled maintenance. - For any additional work done for the fire department,they will re ive a 10%discount,with the exception of subcontractors.
