CITY OF JEFFERSONVILLE
SAFETY EYEWEAR PROGRAM
THIS AGREEMENT is made effective beginning the 1st day of July
2024 between Eye Associates of Southern Indiana located at 1407
Spring Street, Jeffersonville, IN 47130 and the City of Jeffersonville,
with its principal offices located at Jeffersonville City Hall, 500
Quartermaster Court, Jeffersonville, Indiana 47130.
WHEREAS, the City of Jeffersonville desires to contract with Eye
Associates of Southern Indiana (Dr. Black's Office) to operate the City of
Jeffersonville's Safety Eyewear Program for the period set forth below:

WHEREAS, Eye Associates of Southern Indiana desires to
Operate the Safety Eyewear program of the terms and conditions set
forth below:
SECTION 1
Annually, authorized City of Jeffersonville employees are eligible
to receive the following:
a. Safety frame "Basic Collection"
b. One pair of polycarbonate single vision or standard line
bifocal lenses
c. One pair of removable side-shields
SECTION 2
The City of Jeffersonville will not be responsible for payment on
any service or material not listed previously in Section 1 of this
agreement.
SECTION 3
The City of Jeffersonville will allow Eye Associates of Southern
Indiana to make presentations and display information regarding the
Safety Eyewear Program.
SECTION 4
The City of Jeffersonville will not participate in any other Safety
Eyewear Program.
FEES:
Fees for materials/services provided by Eye Associates of Southern
Indian to the City of Jeffersonville will be in accordance with the fee
schedule below:
a) Basic Package — Single Vision $79.00 per package
b) Basic Package — Bifocal $99.00 per package
FEE CHANGES:
The fees for services rendered under this agreement will not change
unless Eye Associates of Southern Indiana notifies the City of
Jeffersonville in writing sixty (60) days in advance of a price change. If
the City of Jeffersonville does not agree to the new price, Eye
Associates of Southern Indiana, at their own discretion, may continue
to provide agreed upon services at the then current price for the
duration of the agreement, or may discontinue the provision of services
on the date the new schedule of fees would take effect, subject to
sever ability provisions described elsewhere in this agreement.
PAYMENT:
Eye Associates of Southern Indiana will invoice the City of Jeffersonville
for all services provided on a monthly basis. Payment of invoices for
each month will be due by the end of the following month. If the City
of Jeffersonville fails to make payments as agreed upon in this contact,
Eye Associates of Southern Indiana may continue to perform the
obligations as per this contract and is entitled to recover all payments
for materials/services rendered.
TERM:
This agreement shall be in effect from the date of execution for a
period of one (1) year. The terms of this agreement will automatically
renew on January 1st of each year and be in effect for a period of one
(1) year, unless notification, in writing, to discontinue this agreement
for the next calendar year is received no less than thirty (30) days prior
to the end of the current agreement. The responsibilities, obligations
and liabilities shall survive the term of this agreement.
IN WITNESS WHEREOF, the parties hereto have caused this agreement
to be executed as of the day and year executed below.
