For Clerk's Office to fill out Date uploaded to Gateway: Kj .t ARTISAN AGREEMENT JEFFE ° I Falls Landing Park: Contracted Sculpture: Contractor: Joe Autry I Joe Autry<autryjoe©yahoo.com> 1502-727-6845 TERMS OF AGREEMENT: The contractor chosen agrees to the following: • Develop and Fabricate a series of 1 sculptures for installation in Jeffersonville based upon the agreed-upon images and Follow the awarded design which was submitted and chosen by the adjudication panel • Create a piece(s)that must remain between 6-8'tall and no more than 3'at the widest point • Discuss and receive approval for any major design alterations to the sculptures during fabrication (including dimension,color etc.) • Take care to consider that the piece must be safe for public viewing and interaction (no hazardous elements) • Provide a colorful piece that has been coated with appropriate exterior paint or powder coated material • Follow the provided paper template for the baseplate (this is essential to a successful install-I will mail this) • Create the piece on a metal baseplate that is 3/8"to 1"thick • Provide a title for the nameplate • The contractor will coordinate delivery with the Public Art Administrator and the contractor will Deliver the piece to the designated location by the installation date chosen in June/July TBD (anchor bolts will be provided) • Provide the city with a W9 (Public ArtAdmin will email this to you if you do not have one readily available) • Lead installation of the finished sculptures,Complete the project(to an acceptable level of technical skill)by TBA June/JULy Install: The timeline maybe adjusted at the discretion of the Public Art Administrator in discussion with the contractor. • Maintain dialogue about the design/fabrication with the Public Art Administrator. • Release any ownership of the finished art,upon completion of the contract • Provide the city with an invoice for services rendered as soon as possible: • The stipend will be$12,000 for services rendered I The payment of$12,000 will be made in 2 installments,the first of$6,000 as an advance for materials and the remaining$6,000 after the successful completion of the performances and activities described above.Check to be mailed to the contractor no later than three weeks after the successful completion to be determined by the Public Art Administrator. The city agrees to the following: • Provide access to the Public Art Administrator during the time period of the contract • Artistic credit in web,print,and media materials • A nameplate giving artistic credit for the work INDEMNIFICATION; The contractor agrees to indemnify and hold the City of Jeffersonville and its officers,agents,and employees,harmless from all suits,claims,or damages,including reasonable attorney's fees arising out of or resulting from activities under this agreement conducted by the Contractor or Contractor's agents.The provisions of this section shall survive the expiration or earlier termination of this agreement.
