The agreement will begin in July 2023 under the terms and conditions below.The agreed upon pricing is noted next to each description. Terms and Agreement 1. Job descriptions for Quarterly Maintenance Station#2 2204 10th Street Jeffersonville, IN 47130 Every 3 months Jet Kitchen sink line and treat lines- $30 Station#5 2006 Allison Lane Jeffersonville, IN 47130 Every 4 months Clean Line under mens lavatory through cleanout-$350 Every 4 months Clean Kitchen sink drain through cleanout- $350 Station#3 1601 Truckers Blvd Jeffersonville, IN 47130 Every 4 months Jet Kitchen sink line-$250 Every 4 months Jet urinal- $250 Station#1 735 Wall St. Jeffersonville, IN 47130 Every 4 months Clean main line under men's sink through clean out- $300 Station#4 5311 E. 10th St. Jeffersonville, IN 47130 Every 4 months Clean Kitchen Sink Line- $250 2.Term This agreement will begins July 2023 and continue through July 2025. At this time the parties will reexamine the agreement to see if any changes want to be made. 4. Rights and Obligations of the Parties (A)The Customer shall not pay additional labor or service fees for the periodic maintenance of the above work described. However, in case of part replacement/change, the cost of these parts shall be paid by the Customer. (B) Damages that may arise as a result of external factors such as fire, flood, and third parties' intervention in the described work areas are not subject to this Agreement. (C) Brian Scroggins Plumbing will prepare an invoice containing a detailed record of the operations performed during each quarterly maintenance and give a copy to the customer. 5) Miscellaneous (A)This Agreement and Maintenance Schedule(s)constitutes the complete understanding of the parties to this Agreement regarding the subject matter contained in this Agreement and supersedes any and all other agreements or arrangements, either oral or in writing. (B) This Agreement and all the rights of the parties hereunder shall be governed by, construed and enforced in accordance with the laws of the State of Indiana. (C) In the event either party wishes to terminate this agreement, they will need to notify the other party within 30 days of the next scheduled maintenance. (D) For any additional work done for the Fire Department, they will receive a 10% discount. By signing this document you are stating you agree to the above terms and conditions.
