upkeep your entire occupied facility to maintain a clean and healthy
environment. We will provide appropriate scheduling to ensure that
the facility is receiving the proper cleanings at the appropriate times.
General Cleaning per scope of work to include hand cleaning the
entire area that is being serviced,with CDC-approved disinfectant
with COVID-19 kill claim.

o Dust/Clean/Disinfect all horizontal surfaces;counters,tables,chairs, phones, microwaves,offices, conference rooms,

o Empty all garbage cans inside the facility. Put new liners in as needed.

This document warrants that should a defect in workmanship, related to the work completed by Healthiana Plus. LLC
occur within 1 year of construction or renovation project, Healthiana Plus. LLC will complete repairs within the original
project's scope of work at no charge to the customer. This warranty does not cover normal wear and tear, hail
damage,wind damage, sun damage. intentional or accidental damage by any person: or acts of God that may or may
not merit an insurance claim. This warranty only applies to portions of the project in which Healthiana Plus. LLC fully
replaced any existing products, and does not cover repairs or service done to another contractor's work.
Defects in the building materials used to complete work do not fall under the scope of this workmanship warranty. any
building products installed will instead be covered by the product's original limited product warranty provided by that
manufacturer for the product.
For specialty services this document additionally warrants the satisfaction of the customer for services offered in
janitorial services. The customer has 24hours to call with his or her concerns of any services performed that was not to
their satisfaction within reasonable limits of service purchased and paid for, and our company will come out and corrcet
the service to their satisfaction.within reasonable limits.

-Contract is set on a yearly contras with monthly payments,with 30-day cancellation notice before auto-renewal on the anniversary date Price will be tilled monthly on a set
salary The monthly budget has beer set to include paid holidays and expected closures There are no reductions in price due to being dosed for Holidays inclement weather.etc.
Pnce will not change if company doses for any pre-set days,holidays.bad weather.or scheduled closings as workers are paid on a salary for the job with sick days vacation days
and paid holidays as well.Exceptions would be unforeseen extended circumstances.such as a pandemic,that closes the company for an extended time in which time there will be
a"pause”or a"suspension hold"on the account.The customer will be given 1-3 months.depending on circumstances.at which time either their payments will auto-start,or they
can request an additional 1-3 months"pause"Once the account resumes,the 1 year contract starts over and it the company has made any changes within the building,such as
renovations that will increase the work load by way of increased staffing,more square footage changing flooring from carpet to hard surface.etc,then the company will agree to a
reasonable comparable price increase.
-If there is additional work needed after onsrte,then this will be discussed with customer and a change order will be made and signed.

—If customer chooses any form of machine cleaning.such as auto-scrubbing,carpet cleaning,stnp 8 wax,pressure washing.window washing,or what is deemed needed at the
time by our technician on specified floors,or any similar project thereof,though we always try our best,sometimes it is not possible to remove all stains and we cannot guarantee all
spots and dirt will come up from carpet or hard surfaces-We are not responsible for damages or loose tiles.carpet,or flooring peeling or chipping paint breakdowr it sea.ant.or
electrica'failure or other similar circumstances that may result from any machine cleaning.-We are not responsible for wear and tear of vinyl layer of LVP that may occur from
machine cleaning and daily use.
—Unless otherwise specified.the price includes minimal fixtures that can be easily moved without the use of a dolly.to be moved for cleaning floors Customer waives any
responsibility or liability on our side for the damages that may occur to the furniture or fixtures or flooring during the course of the job.
-We will dean all accessible areas of carpet or hard surface flooring.Includes all main areas and walkways Includes moving minimal furniture,such as chairs,trash cans,etc Does
not include moving large fumrture,such as desks,filing cabinets and heavy tables.These areas can be included for a furniture moving fee.'Customer will be responsible for moving
all Small items,all personal belongings.all electronics. We will not unplug.disconnect,or reconnect anything.(i.e.fax machines.phones.computers.TVs)
-This price does not include moving the large equipment or shelving.or bolted fixtures.This can be included for an additional fee.
-If waxing-After all cleaning is finished,customer will agree as to where all fixtures will be placed before waxing starts.We will not return to move any fixtures after waxing is
finished.Any fixtures not in the proper place will be the responsibility of the customer to move after we have completed Me waxing.We can return for furniture moving for a nominal
fee.Please allow 24 hours to cure before moving any heavy items on floors.

inspected and accepted by customer.If client agreement is terminated but services have been partially performed.client is obligated to prorate payment of the Compensation
equivalent to work done,The Contractor agrees to not divulge or reveal and confidential information that the contractor obtains through the course of business.Contractor maintains
the nght,at own discretion,to engage third party sub-contractors for the purpose of performing some or all of agreed work that fulfills this contract.Customer agrees to not hire a
third party to assist or aid in the completion of services.If contractor chooses to hire a sub-contractor,contractor will pay for services performed to sub and the amount agreed upon
in invoice is still payable from client to contractor.Contractor has full autonomy of work methods,time spent,and the decisions for the purpose of accomplishing a task in relations to
the contract and not at the guidance or direction of client.Customer to pick out and/or approve all items and work.Purchase and/or install of items.or commencement of work
constitutes customers agreement and approval of item and/or services.Contractor or sub-contractor wilt provide any and all of the tools necessary for the completion of the project

We do not guarantee that all windows will be perfectly clean.It is understood that windows can become dirty again during construction and we are not responsible for recleaning without an additional cost.
WHEN A JOB IS ABLE TO BE PAID OUT IN PHASES-WE DO NOT ACCEPT SHARING PRICES AND CLEANS WITH ANOTHER CONTRACTOR,OR REDUCTION IN PRICING IF A CUSTOMER
HIRES ANOTHER CONTRACTOR.'When cleaning apartment complexes,price is paid according to full square footage of POD,except for what entire unit a prior cleaners has done.There is not a price
reduction if contractor decides a particular unit does not need a particular stage of clean,for any reason.(i.e.our workers performed such a complete and thorough job on the rough or final that the powder
puff is no longer needed.)
ADDITIONALLY
•Excludes ceiling and exterior cleaning of building.
•Excludes inside closets,appliances and/or cabinets unless requested.

INDEMNIFICATION

It will be the responsibility of an authorized personnel to either provide a key,or make this area available to our cleaning team immediately upon arrival to the jobsite for service to be performed.if we were
not provided a key to gain access.Any delay in our contractor or subcontractor(s),upon ariving at the site.in their ability to gain access or to perform their work,will result in a 10%penalty fee,minimum
of$69,per hour per scheduled worker.as all workers are paid for the time to drive to the jobsite and on the clock while awaiting to gain access to area being serviced.Delays include but not limited to:not
having all materials agreed upon available,delay of allowing workers to gain access to area work is to be performed.(such as not unlocking area to be serviced or not being reedy for our services,and we
have to reschedule,after we have already arrived on site),delay of payment,or refusal of owner,or authorized representative to allow work to commence.
