Monthly Service
Preventative Maintenance Contract
ATOM CHEMICAL INC. AND CUSTOMER AGREE TO THE FOLLOWING:
SECTION A-
1. Provide necessary water treatment chemicals and maintain proper chemical residuals to
control and minimize scale, corrosion, biological growth, and foaming.
2. We will provide the following service and testing:
A.)Test/check all necessary chemical residuals.
B. Clean all chemical feed pumps, foot valves, and injectors as needed.
C. Calibrate and adjust all chemical feed equipment as needed.
D. Inspect all chemical feed equipment for any leaks and recommend any necessary
repairs or equipment upgrades.
E. Make sure all pumps are primed and function properly.
F. Provide a digital service report detailing chemical residuals, necessary repairs,
recommendations, and customer signature at time of service.
3. Inspection and cleaning of all filter housing and filter cartridges purchased from Atom
Chemical, Inc.
4. Provide labor, chemicals, and equipment to remove excessive scale or
deposits acquired during our treatment program; provided that our program and
recommendations are implemented with reasonable care.
5. All testing chemicals and freight.
6. Training seminars for your staff upon request.
7. Provide emergency service upon request.
8. Delivery of chemicals to the point of use.
9. Removal of all empty Atom Chemical, Inc. chemical containers each service visit.
SECTION S-
NOT INCLUDED IN THIS CONTRACT:
1. Waste of chemical due to negligence.
2. Responsibility for pre-existing conditions.
3. Re-treatment of closed systems, hot/chilled loops, and/or geothermal systems
due to leaks, repairs, or loss of water.
4. Glycol products.
5. Repair or replacement of any piping or related equipment damaged from freezing,
6. Repair or replacement of any piping or related equipment damaged from freezing due to
inadequate glycol levels.
7. Repair or maintenance of heat trace.
8. Filter cartridges.
9. Salt
10. Cleaning of cooling tower
11. Service and labor to clean cooling tower after hours, weekends, or on a holiday.

12. Any condition or loss caused by or related to a failure of customer's equipment or property or
conduct of customer's employees. Customer agrees to fully defend and indemnify Atom
Chemical for any such loss or claim.
13. Installation of any taps needed for chemical feed equipment.
14. Inspection of steam boilers after hours, on weekends, or holidays.
SECTION C-
CONDITIONS OF THIS CONTRACT:
1. The price quoted is firm for twelve (12) months from the start date.
2. The customer or Atom Chemical, Inc. may cancel this contract for any reason with a thirty(30)
day written notice.
3. Customer will be billed on the first of every month. Payment is net thirty (30)days.
4.Atom Chemical, Inc. reserves the right to apply a 1.5%finance charge for any past due
invoices on the total amount due.
5.Any dispute related to this contract shall be brought exclusively in the Courts in Floyd County,
Indiana.
6. If Atom Chemical, Inc. seeks to collect any unpaid balance owed; it shall be entitled to collect
its reasonable attorney fees.
7. Any special water treatment requirements for the system components and metallurgy must be
disclosed by the owner/customer at the signing of this agreement.

Terms &Conditions
Pricing in this agreement is to remain in effect for(1)one year, unless canceled by either party in
writing giving a (30)thirty day notice. At the end of the service agreement period, the agreement
shall automatically extend for additional year periods unless written notice of cancellation or
modification request is made. Any changes after the (1)one year agreement are to be written out
and a new agreement is to be signed by both parties. Atom Chemical, Inc. reserves the right at
any time to correct clerical errors.
