(CONSULTANT), dated 04/01/2010, this WORK ORDER is the written authorization to the CONSULTANT to provide the work described herein, in accordance with the attached schedule, and fees Work: The Work shall be as described in Section A of this Work Order. CONSULTANT shall furnish all labor, materials, supplies, equipment, supervision and services necessary for and incident to the performance of the Work. CONSULTANT represents that it has thoroughly reviewed the Work and the General Services Agreement and that it accepts the Work and the conditions under which the Work is to be performed. Schedule: The Schedule shall be set forth in Section B of this Work Order. CONSULTANT represents that the Schedule is reasonable and achievable. Payment Terms: Payments to the CONSULTANT shall be as described in Section C of this Work Order. Terms and Conditions. The terms and conditions of the General Services Agreement referenced above shall apply to this Work Order. This Work Order also incorporates all of the terms and conditions required to be included in it by the General Services Agreement. (JTL)shall provide Professional Engineering services for the project described as follows: JTL will be responsible for preparing construction plans and technical specifications for the City of Jeffersonville to construct a building and concrete pad for a storage container for their phosphorous removal system. The building will include any necessary lighting, heating, ventilation, small power and instrumentation. JTL will evaluate the cost benefits of using a double walled storage tank for the phosphorous removal chemical versus a containment chamber as part of the slab. JTL will assist the department with submittals to IDEM to obtain approval for permitting the project as a Wastewater Treatment Plant Construction per 327 IAC 3. Deliverables will include plans and specifications for the building and storage container pad as well as application preparation and submittal to IDEM. Assumptions: • Adequate power is available nearby for the new equipment as well as connection to the existing SCADA system. • The Owner will provide programming of the existing PLC and HMI by the Owner's system integrator. • There are no utilities under the proposed building site and pad. • The Owner anticipates constructing the phosphorous removal system upgrades in house. SECTION B— SCHEDULE Plans and Specifications for the building and storage container pad will be complete within approximately 60 days, subject to Owner review. The IDEM Permit application can be submitted at the completion of Plans and Specifications and review of the permit application may take 30-90 days. SECTION C — PAYMENT TERMS Payment to the CONSULTANT for Professional Engineering Services shall be for a lump sum fee of $19,000.00.
