Request for Change Order#1
Dear Mr. Darby;
Per our earlier requests and conversations,TSI hereby requests a Change Order for the following
revisions and changes in scope of work on the above referenced project. Please note the following items:
A.
• Revisions in pedestrian bridge supports. TSI proposes to eliminate the poured in place piers,
including the excavation,backfill, erosion control, shoring, etc,and replace it with a driven pile
alternate. This includes the following:
• Stamped engineering drawings. (Indiana Stamp).
• HP 12x53 piles with points driven with vibratory hammer on excavator. (4) piles each bent, (2)
battered and (2)vertical.
• Reinforced concrete bents+/-2 ft wide x 3 ft tall x 16 ft long.
• Set bridge with bearings as shown on original drawings.
• Pour deck as shown on original drawings
If approved,the Indiana P.E. will provide the engineered drawings to JTL for final approval.
Total Credit for this item---$21,000
B.
Change the Keystone type walls with geogrid reinforcing to a stand alone Redi-Rock design(approved
by JTL prior to Bid). Locations,elevations, etc,remain the same. A complete submittal on this type of
wall has been attached to this letter.

2 Delete Quantity from Item No.26 HMA Over Concrete Pavement Patch:150 SY @$80.00/SY = $ (12,000.00)
3 Revise Prefabricated Pedestrian Bridge Foundation System = $ (21,000.00)
TOTAL = $ (88,600.00)
The method of payment for this change or extra work shall be:
