Court Avenue in Jeffersonville. The purpose of the project is to permanently abandon up to four (4) groundwater monitoring wells located on the Subject Site or in the City right-of-way (RoW) on the perimeter of the Subject Site. Currently one (1) monitoring well (OW-39) is located in the central portion of the Subject Site, a location that will almost certainly be disturbed/destroyed and/or covered by a building footprint during the planned redevelopment of the property. The remaining three (3) monitoring wells (Duke MW-16, MW-17, and MW-18) are located in the City RoW on the periphery of the Subject Site in locations that may or may not be significantly impacted during the site redevelopment. Supplemental Phase ll ESA For this task, BCA will contract an Indiana licensed well driller to abandon up to four (4) monitoring wells according to all local, state, and federal requirements. This will include pulling the well(s) and plugging with bentonite. If required, the driller will remove the existing concrete pad/manhole and will restore the surface. Finally, the driller will send a record of the well abandonment(s) and plugging procedures to the IDNR Division of Water. 7202 E 87th St.,Suite 110 224 W Jefferson Blvd,Suite 204 Indianapolis,IN 46256 South Bend,IN 46601 Phone(317)578-4233 Phone(574)522-1019 Fax(317)578-4250 Fax(574)522-0374 www.bcaconsultants.corn The estimated cost for abandoning all four (4) wells (OW-39, MW-16, MW-17, and MW-18) and restoring to grade is $2,200. If only well OW-39 is abandoned, then the estimated cost would be $850. If this project can be performed in conjunction with the Limited Soil Delineation sampling project being planned for the American Legion site, then the quoted costs above will be reduced by $115 due to combined mobilization costs. The project will be billed monthly on a percent completion basis. Our current schedule allows us to start the task immediately upon receiving your approval. Please note that this cost estimate is valid for 120 days. Please return a signed copy of the enclosed Proposal Acceptance Sheet with the appropriate information completed. Thank you for the opportunity to earn your business. Please call me at (317) 578-4233 should you have any questions. Sincerely, Len Hinrichs, LPG Project Manager W a i 5 '...
