SUPPLEMENTAL AGREEMENT NO. 1
PROFESSIONAL ENGINEERING SERVICES
DESIGN/CONSTRUCTION INSPECTION
This is a Supplemental Agreement made as of this 16th day of December, 2003, between the City of
Jeffersonville, Indiana, acting thru its duly appointed Drainage Board (CLIENT) and Jacobi, Toombs, and
Lanz, Inc. (JTL), a firm of Professional Engineers, Surveyors and GIS Specialists.
WHEREAS, CLIENT and JTL entered into a Professional Engineering Services Agreement on October 21,2003,
and
WHEREAS, the original agreement failed to recite a monetary budget for full-time construction inspection, which
is part of said original agreement,
NOW, THEREFORE, the CLIENT and JTL agree to the following changes and supplements to said ongoing
Professional Engineering Services Agreement dated October 31; 2003:
Attachment 'B' is hereby changed or supplemented as per enclosed revised attachment.
Unless changed or supplemented by this Supplemental Agreement No. 1, all other terms and conditions of the
original agreement dated October 21, 2003, shall remain in full force and effect.
In witness whereof, the parties hereto have made and executed this Supplemental Agreement as of the day and
year first written. >j

ATTACHMENT B
FEE COMPUTATION SUMMARY
DESIGN
Collection of information, property data, locate property corners, prepare map. $
Set elevation bench marks, take elevations, plot all data on large plan, work out $
preliminary design of how area would be drained and present for approval of City.
Prepare final design of sanitary sewers, storm sewers, streets, driveway entrances.
Prepare plans, specifications and bid documents for bidding purposes. Present for
final approval.
Comptete any necessary revisions, publish notice, distribute plans to bidders, attend $
bid opening, award and process contractors documents.
During construction, observe contractor, handle questions and Work out problems $
that arise.

During construction, provide one (1) full time inspector to ascertain that the project
is constructed in accordance with the plans, specifications, construction documents
and construction contract.
Compensation based on a time and materials basis, in accordance with Attachment
'D' of the original agreement, with a not to exceed budget of
$ 100,000.00
