G, ../i The Shirley Hall Youth Football Shelter 2 May 2007 Section One Proiect Scope 1.01 The scope of this project is limited to work at Shirley Hall Park, located near the intersection of Charlestown Pike and Dutchman's Lane in Jeffersonville, IN. 1.02 The Shirley H.all Youth Football Shelter is defined as: Size: Approximately 1,800 Square Feet of New Construction Location: Property located on the grounds of Shirley Hall Park The shelter will consist of a free standing CMU building with a standing seam metal roof. Components of the project will consist of: - Multi-unit public restrooms - Picnic seating - Concessions - Concession's storage - Janitors closet - Electrical closet - Large equipment storage Scope: 1.03 The probable total budget for this project is estimated to be $185,000. The Shirley Hall Youth Football Shelter 2 May 2007 Section Two Scope of Professional Services 2.01 The Scope of Services includes the following: .1 Architecture .2 Mechanical, Plumbing and Electrical Engineering Services .3 Structural Engineering Services .4 Civil Engineering Services NOTE: The scope of services does not include fees for site survey, land acquisition, geotechnical report, communications and low voltage systems planning, furniture, furnishings, office equipment, art work, signage, window treatment, etc. These are the responsibility of the owner/facility. The Estopinal Group can provide these additional services and would be pleased to provide assistance under a separate contract if desired by the facility. 2.02 Project Development Phases will include: .1 Schematic Design .2 Design Development .3 Contract Document Development .4 Bidding and Negotiations .5 Contract Administration during construction 2.03 Schematic Design will include: .1 Review the existing site plans/preliminary designs .2 Field verification of existing site conditions .3 Develop floor plans to indicate room adjacencies; public and staff flows; space sizes, etc. .4 One( 1) meeting with the Owner to review the proposed Schematic Design .5 Develop a Schematic Design Package for the Owner's approval and sign-off .6 Develop an anticipated construction cost estimate r \ , "~i 2.04 2.05 ~'i / ;;;,/ Design Development will include: .1 One(1) meeting(s) with the Owner to determine the locations and requirements for the following elements: .a Plan Layout(s) .b Casework .c Types of Finishes .d Doors and Hardware .e Mechanical Systems including choice of system type .f Plumbing Systems including plumbing fixtures .9 Electrical Systems including light fixtures and device locations .2 Develop a Design Development Package for the Owner's approval and sign-off .3 The Design Development Package will then be distributed to the project team for use in developing the Contract Documents. Contract Documents will include: .1 Cover Sheet .2 Project Reference Sheet .3 Civil Drawings .4 Structural Drawings .5 Architectural Site Plan .6 Life Safety Composite Plan .7 Dimension Plan .8 Architectural Plan .9 Reflected Ceiling Plan .10 Roof Plan .11 Exterior Elevations .12 Wall Sections .13 Plan details .14 I nterior Elevations .15 Casework Sections .16 Miscellaneous Details .17 Opening Schedule The Shirley Hall Youth Football Shelter 2 May 2007 .18 Opening Details .19 Room Finish Schedule .20 Mechanical, Plumbing and Electrical System Drawings .21 Project Specifications . 2.06 Bidding and Negotiations: .1 TEG will assist in bidding the project via public bid. The City of Jeffersonville will provide all legal documents and Instructions to Bidders pertaining to the Bid. .2 TEG will conduct One(1) on-site Pre-Bid Conference with the interested bidders. .3 TEG will assist the Owner in tabulating and analyzing the bids. .4 TEG will provide copies of the standard AlA Form of Agreement between Owner and General Contractor for the Owner's legal counsels' use in the development of their contractual agreement. 2.07 Contract Administrative Services will include: .1 The Project Team will provide shop drawing and submittal reviews. .2 The Project Team will review all Applications and Certificates for Payment from the General Contractor. .3 The Project Team will conduct one (1) on-site visit per month and coordinate project progress meetings with the General Contractor and subcontractors on tl)e days of the on-site visits. .4 The Project Team will prepare a final project Punch List review substantial completion prior to certification of the final Application for Payment. 2.08 Electrical Engineering Design Services will be limited to the specification of power and lighting systems and the low voltage systems infrastructure. Note: The low voltage systems infrastructure shall be defined as back boxes, conduit stub-ups, and cable management raceways only. With exception of the fire alarm system which shall be contractor furnished and contractor installed; the selection, purchase and installation of all other low voltage systems (i.e. telephone, data, nurse call, public address, intercom, CATV, MATV, SATV, CCTV, security, infant abduction, computer and hospital information systems, radio systems, dictation, transcription, time and attendance systems, etc.) including cabling and hardware, shall be the responsibility of the owner/client unless otherwise specifically indicated in this proposal. . r, ".-' i The Estopinal Group can provide these additional services and would be pleased to provide assistance under a separate contract if desired by the owner/client. 2.09 Interior Design Services will be limited to the specification of types of interior materials and their locations. The selection of colors, patterns, fabrics, furniture, furnishings, office equipment, artwork, signage, window treatments, are not included in this proposal. These services are the responsibility of the owner/facility. The Estopinal Group can provide these additional services and would be pleased to provide assistance under a separate contract if desired by the facility. 2.10 This proposal does not include any hazardous material abatement services. If any hazardous materials are encountered, it is the owner's responsibility to coordinate assessment and abatement services. 2.11 This proposal is based on the following Project Schedule: Initial Project Meeting June 13th, 2007 Schematic Design Phase 1 week Design Development Phase 2 weeks Contract Documents Phase 3 weeks CD's released to Bidders Pre-Bid Meeting Bids Due 3 weeks Start of Construction August 15th, 2007 The Shirley Hall Youth Football Shelter 2 May 2007 r'i Section Three Basis for Compensation 3.01 The Estopinal Group presents a fixed fee of $12,000 (Twelve thousand, five hundred) for the services outlined in this document. 3.02 Reimbursable Expenses are not included in the fee stated in 3.01. Our firm's policy on reimbursable expenses is to invoice our clients at our direct cost with no mark-up for processing. Examples of reimbursable expenses include but are not limited to the following: - Travel Expenses (mileage based on $0.485 per mile) - Reprographic Expenses for the production of check sets and bidding sets. - Reprographic Expenses for the production of presentation drawings and display boards. - Shipping Expenses associated with distribution of materials associated with the project. - Advertising expenses - All City, State and other Authorities having jurisdiction Plan Review Fees, Zoning Applications, etc. 3.03 Invoices will be distributed on a monthly basis. All invoices unpaid after thirty (30) days beyond the invoice date are subject to interest charges at a rate of 1.5% per month applied to the outstanding balance. 3.04 If for any reason, the project is halted for more than 14 calendar days; The Estopinal Group is to be compensated for all work and expenses performed prior to the stoppage. r) "",' (\ ".... ;;,~/ The Shirley Hall Youth Football Shelter 2 May 2007 Section Four Additional Services 4.01 All services beyond those outlined in this Proposal are considered Additional Services. If Additional Services are required, a separate proposal defining the Scope of Services and Professional Fees will be prepared 4.02 Revisions to designs or documents after approval at each project development phase or final documents previously approved by the Owner/Owner's Representative. 4.03 Meetings beyond the meetings outlined in the Scope of Services, (including meetings with city, state or other agencies for review of the project.) 4.04 Revisions to drawings upon completion of constructionlinstallation to provide accurate as-built drawings. This proposal is valid for 30 days from date issued. The Estopinal Group LLC's contractual obligation is complete on December 31, 2007. Thank you for your consideration of our Proposal. Evaluating professional services is very difficult, scope of services and fees vary greatly from one proposal to another. If services outlined in this Proposal are not as you desire or do not reflect the scope that others may propose, please allow The Estopinal Group to modify to better serve you and the project. -' ~ji.
