Dear Robert:
In response to your Request for Price Quotation on the above referenced project, we
are pleased to offer the following proposal and scope of work. Please note that we are
trying to provide the Owner an accurate top end cost, based upon the existing site
conditions, and our expectations as to what might occur.
Please note our responses and scope based upon your numbered items.
1. Mob & Demob is included, complete.
2. Dewatering well-Your RFP requests us to include one well, which we can price.
However, in trying to provide accurate information, we have included the costs of
3-4 wells. We have been in contact with Kelley Dewatering, Griffin Dewatering,
and Hydraserve, all noted dewatering firms. Kelley is very familiar with this
project and has been onsite. They most recently bid the WWTP and Louise St.
projects and have extensive soil borings. Both Kelly and Griffin have advised
that one well will not get the water level down and that a minimum of 3, and they
both recommend 4 wells will be required to get the water down to the necessary
level. You may contact Brad Kelley at 616-340-2038. An additional problem, as
pointed out by both dewatering companies, the Ohio River Upper Gauge is very
high with only a small predicted drop for the next 3 weeks according to the US
Army Corps of Engineers Website, which increases the amount of dewatering.
Our lump sum bid includes the cost of a maximum of 4 wells for the 30 day
period. If you want a price for only one well, we can provide that option.
3. All excavation, haulofftrucking, sheeting, and trench boxes are included. Due to
existing utilities, groundwater, etc, the only feasible method is to utilize the
United Slide Rail Shoring System, which we have included.
4. We have included 24' of 60" RCP, due to the standard 8' lengths. Independent
Concrete Pipe has advised us that they only run 4' sections when it involves a
large number of pieces and will not tool up fo 1-4' section. Concrete collars and
tie-ins to existing pipe, structure, etc. is included.
5. Granular backfill oflndiana #11 is included.
6. We have included the paving spec as detailed as 12" dga, 4" HMA Base, 2" HMA
binder, and 1" HMA surface. Please note that as ofthis morning, the actual

cracks are 60' apart, and we are assuming that an additional 10' on each end has
already been undercut and will need to be replaced. Our bid is based upon
replacing 80 If long x 22' wide asphalt patch. This also includes the excavation of
this area and refill with # 11 stone to the necessary depths, which we anticipate to
be 10-12'.
7. We have included tying in the pipe to the new manhole. However, within the last
3 days, the top of the manhole grate is now almost a foot below the surface which
shows that the manhole has settled as well. This will involve more than
"stabilizing" and will probably result in that manhole being removed and either
reinstalled or replaced, once the excavation and extent of the manhole damage is
uncovered. Any costs to remove, replace, or relocate the manhole are not
included.
8. We have included 160 If of curb, which reflects the 80' of roadway estimated to
be replaced.
9. Surface restoration included in this bid includes the removal ofthe two big trees
(one is already falling and the other will probably be lost as well), recompacting
and filling the two yard areas that have settled almost a foot (but not any
excavation), seed, etc. The one asphalt driveway is cracked and settled 16' from
the edge of curb and we have included replacing that drive, 20' x 8' wide. In
addition, the two exposed aggregate drives will have at least their aprons replaced
as they have settled as well. All of that work is included in our base bid.
10. Traffic control as required is included, including orange snow fence completely
around the perimeter of the construction area during non-working hours.
11. Cleaning and jetting of the existing storm sewer to a maximum of 120 lfis
included.
NOT INCLUDED IN BASE BID
1. Any allowances to replace or temporarily cut and remove the existing water main
that runs parallel to the storm pipe.
2. Any allowances to replace or temporily cut and remove the existing gas, cable tv,
or sanitary sewer property leads in the affected area.
3. There is at least one existing curb basin inlet that has already settled as well and
we assume the lead pipe to it has also settled. We include no costs at this time to
raise or repair that basin or pipe lead until such time as it is excavated and the
necessary repairs are determined.
4. No bypass pumping of the existing storm piping is included.

ADDITIONAL UNIT PRICES:
1. 60" RCP-If pipe can be added without additional dewaterin,g wells or asphalt
replacement~ the unit price will be $2,100 per Jf. Based upon discussion with our
dewatering sub, we think that a maximum of an additional 16' (2 joints) of 60" can
be installed utilizing the dewatering system for the first 24'. Also, as we have
included replacing 80' of asphalt in out' base bid, this increase of pipe should not
cause any additional asphalt replacement.
2. 60" RCP----If pipe can be added into the existing excavated pit, without additional
dewatering, hauloff, etcJ then the unit price in item 1 will be lowered to $ 1,110.
This will include only moving the sheeting box, excavating down to the flowUne
from the previously excavated pit (approximately an addinona16'), removal of the
existing pipe, and placement of the new pipe. This includes no allowances for
additional stone refill, asphalt patching~ existing utility repairJ dewatering, or
extended rental on the shoring system. The decision to replace additional pipe must
be made as soon as the condition of the existing pipe is determined.
3. 60" Rep-If additional wells are required, then an addi.ti.onal $200 per If will be
required, in addition to more contract time if it requires the dewatering sub to
remobiJi.ze to the site, drill the wells, and then wait for the water to be putled down
to a manageable level.
4. We have included 80' x 22' offull depth and full width asphalt replacement in our
bid. If the road is undercut worse than anticipated~ or if additional pipe or other
utility repair is required, our additional price to place the dgabase and asphalt, will
be $60.00 per sy.
5. We have included the dig out, hauloff., and dump fees for the in place material in
order to rebuild the street. Again, this is based upon our 80' x 22' parameters. If
additional excavation is required, beyond this area~ please add $16.00 per cy.
6. We have included necessary refill of # 11 stone in. the affected area of new asphalt.
If additional excavation is required, then the cost to refill those areas with # 11 stone
will be $12.70 per ton, compacted in place.
7 . We have included 160 If of new curb in our base bid. Additional curb will. be
$24.00 per If.
8. Concrete driveways or aprons, beyond the two included in our bid, will be
additional at $46.00 per sy. Asphalt driveway restorati.on will be $24.00 per sy.
This should cover the bulk of any additional work that might be required and establish
those units now. I do not know if we can cover all ofthe unknowns but we have tried to
eliminate the bulk of the costs in our lump sum bid. If you have any questions, please
