1.0 EFFLUENT QUALITY
During December, effluent quality was within NPDES permit limits for BOD and TSS
concentrations. Table 1.1 summarizes the effluent quality data. Attachment A contains Time
Series Plots of daily BOD and TSS values.

2.0 FACILITY OPERATIONS
Attachment B contains a list of septic haulers that discharged to the Facility during
December.
During December the treatment process within the facility continued to perform excellent.
We continue to train the operators in monitoring and controlling equipment run times in order
to reduce electrical usage. With our energy management program we should realize
significant cost savings.

One problem did occur in December that had an affect on the facility. A shear pin that
connects the operating shaft to the No.2 Pinal Clarifier drain valve failed. This will not
allow us to place the clarifier in operation. Weare in the process of receiving bids to make
the necessary repairs. We anticipate the repairs to be completed by late February.
2.1 PRETREATMENT
During December there were two activities related to the pretreatment program. Voss Clark
Industries received a Notice of Violation for accedence of permit limits for arsenic
concentrations. Voss Clark has made a piping change within their process to reduce flow to
an equalization tank. We will continue working with Voss Clark to resolve this problem.
Industrial Water Recycling (IWR) requested a modification to their permit concerning daily
average flow limits. IWR requested the limits to be increased from .05 mgd to .1 mgd. The
increase in flow was approved.
3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
Preventive maintenance was performed on all equipment as scheduled during December.
There were 20 unscheduled maintenance tasks performed. All were minor tasks except for:
. Repair of No.3 blower motor at solids handling.
. Repair of No.1 process water pump.
. Replaced No.2 pump at Mill Creek lift station.
. Repair of No.2 pump motor at Mill Creek lift station.
A list of unscheduled maintenance work orders is included as Attachment C.
Maintenance and repair expenditures for the month of December are detailed in Attachffi4~nt
D. Table 3.1 represents the total amount. expended in December and for the year. Table 3.2
includes the same information for repair and replacement expenditures.

4.0 FACILITY SAFETY AND TRAINING
Safety inspections were conducted on December 15, 1994. The rating was 96%. The
deficiencies reported were:
. Fiberglass ladder is not available for working on electrical equipment.
. Proper storage of equipment and supplies, store room was cluttered.
. Knowledge of proper response to an accidental spill needs improvement.
. All areas of the Safety Program have not been addressed with the employees.
A copy of the Safety Inspection Report is contained as Attachment F.
On December 21, 1994 a review of all the safety equipment and its proper use, was held at
the facility.
Waymon Payne was appointed the Safety .Coordinator for the facility.
5.0 SEWER COLLECTION SYSTEM
During the month there were 19 sewer calls. Ten of these calls regarded sewer backups. Six
of the backups were due to blockages in the City's main lines, and two required minor
repairs. Four of the sewer calls required cleaning of catch basins. The health department
notified us of sewage being present in an open ditch. The problem was traced to a trash
dumpster at a local business. We notified the establishment of the problem, and helped in
resolving the concern.
