1.0 EFFLUENT QUALITY
During September, effluent quality was within NPDES permit limits for BOD and TSS
concentrations. Table 1.1 summarizes the effluent. quality data. Attachment A contains Time Series
Plots of daily BOD and TSS values.

2.0 FACILITY OPERATIONS
Attachment B contains a list of septic haulers that discharged during the month of September.
During September, the treatment processes continued to perform excellent. We did hire a new
laboratory technician / operator at the facility. Gina (Becca) Abrams started employment at the facility
on September 11, 1995. Becca is a recent graduate from the University of Louisville.
2.1 PRETREATMENT
During September, one industry reported a violation of their discharge permit. Voss Clark
reported a violation for exceedence of Arsenic lil11.its, and a notice of violation was issued.

Due to the Administrative Show Cause Order issued to Voss Clark for chronic violations of
arsenic conentrations during the period of July 1, 1994 through December 31, 1994, Voss Clark
is required to pay the remaininder of the fine issued against them on May 17, 1995. The amount
of the fine due is $5,500.
We will continue to work with all the industries to ensure compliance within their permit limits.
3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
Preventive maintenance was performed on all equipment as scheduled during the month of
September. There were 12 unscheduled maintenance tasks performed. All were minor repairs
except for:
~ Replacement of a hose coupling to the jet truck.
~ Repair of the carburator to the Ford F-700 dump truck.
~ Replacement of the rear end to the F-150 pick-up.
~ Replacement of the No.2 Pump and valving at Camp Powers lift station. .
~ Replacement of the water level electronic transducer at the Tenth Street lift station.
A copy of the unscheduled maintenance work orders is included as Attachment C.
Maintenance and repair expenditures for the month of September are detailed in Attachment D.
Table 3.1 represents the total amount expended in September, and for the current contract year.
Table 3.2 includes the same information for repair and replacement expenditures. Attachment E
contains a detail of repair and replacement expenditures for the year.
During September there were two repair and replacement projects completed. The first was the
rehabilitation of the Cedarview lift station. The lift station received a complete rehab of the
pumps, valves, and electrical control system. The second project was due to emergency repair
and replacement of the water pump to the facilities emergency generator.

4.0 FACILITY SAFETY & TRAINING
Safety inspections were conducted on September 21, 1995. The rating was 94%. The
deficiencies reported were:
~ All necessary non-sparking tools are not available.
~ Proper liquid flammable storage was not used.
~ Store room was cluttered and unorganized.
~ Oil and grease spills were not cleaned up properly.
~ An extention cord was being used when not in good condition .
~ Electrical contacts at the control panels were not free of dust.
The deficiencies will be addressed and corrected ~s soon as possible. All the necessary
equipment has been ordered to resolve the deficiencies. A copy of the Safety Inspection report is
included as Attachment G.
On September 27, 1995 the Facilities Emergency Response/ Contingency Plan was reviewed
with employees of the facility and Clark Miles, who represented the Jeffersonville Fire
Department.
5.0 SEWER COLLECTION SYSTEM
During the month there were six sewer calls. Two of the calls concerned odor complaints. The
odors originated from the collection system in the area of 200 East Park Place. Two of the calls
were due to blockages within the sewer system and the remaining calls were due to improper
drainage of catch basins. Table 5.1, on the next page, shows the data of September sewer
projects.
