1.0 EFFLUENT QUALITY
During June, effluent quality was within NPDES permit limits for BOD and TSS concentrations. Table 1.1
summarizes the effluent quality data. Attachment A contains Time Series Plots of daily BOD and. TSS
values.

2.0 FACILITY OPERATIONS
Attachment B contains a list of septic haulers that discharged during the month of June.
During June, the treatment processes continued to perform excellent. No major problems occurred
during the month.
2.1 PRETREATMENT
During June, there were two industries in violation of their discharge permits with the City's
pretreatment program. Table 2.2 illustrates each industry, and where they exceeded their limits.

Industrial Water Recycling received an Administrative Compliance Order for improper operation of their
pretreatment facilities and discharging odorous compounds into the City's collection system. The order
requires IWR to be in compliance by July 15, 1995.

3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE

Preventive maintenance was performed on all equipment as scheduled during the month of June. There
were 13 unscheduled maintenance tasks performed. All were minor except for the repair of the
submersible pumps at Crums Lane III lift station, and the repair of the pump motors at Sunset lift station.
A copy of the unscheduled maintenance work orders is included as Attachment C.
Maintenance and repair expenditures for the month of June are detailed in Attachment D. Table 3.1
represents the total amount expended in June, and for the current contract year. Table 3.2 includes the
same information for repair and replacement expenditures. Attachment E contains a detail of repair and
replacement expenditures for the year.

4.0 FACILITY SAFETY & TRAINING
Safety inspections were conducted on June 20, 1995. The rating was 93%. The deficiencies reported
were:
~ Steel toed rubber boots are not available to all employees.
~ All necessary non-sparking tools are not available.
~ Fiberglass ladder is not available for electrical work.
~ Drain covers at the press room were not in place.
~ Store room was cluttered and unorganized.
~ Water hoses were not properly stored and were laying in the walkways.
~ Trash cans were not emptied regularly.
~ Electrical contacts at the control panels were not free of dust.
~ Some of the chemical storage containers were not labeled.
Most of the deficiencies were corrected immediately, and the necessary equipment will be purchased in
July to resolve the deficiencies. A copy of the Safety Inspection report is included as Attachment F.
On June 21, 1995 Hazard Communication training was provided for all employees of the facility. The
training discussed how to interpret material safety data sheets (MSDS), and the proper labeling of
chemical storage containers.
5.0 SEWER COLLECTION SYSTEM
During the month there were 12 sewer calls. Three of the calls concerned odor complaints. Four of the
calls were due to blockages of the main sewer line, and the remaining calls were due to catch basins not
operating properly. Table 5.1, on the next page, shows the data on June's sewer projects.
