for the month of April 1995, containing information on the following:
1.0 Effluent Quality
2.0 Facility Operations
2.1 Pretreatment
3.0 Preventive and Unscheduled Maintenance
3.1 Maintenance & Repair Expenditures
3.2 Repair & Replacement Expenditures
3.3 Electrical Expenditures
4.0 Facility Safety and Training
5.0 Sewer Collection System
As always, we appreciate the opportunity to be of service to the City of Jeffersonville, and we are

1.0 EFFLUENT QUALITY
During April, effluent quality was within NPDES permit limits for BOD and TSS concentrations. Table 1.1
summarizes the effluent quality data. Attachment A contains Time Series Plots of daily BOD and TSS
values.

2.0 FACILITY OPERATIONS
Attachment B contains a list of septic haulers that discharged during the month of April.
During April, the treatment processes continued to perform excellent. We continue to experience
problems with the diffused air system at the solids digester units. The diffusers' air port plugs are not
allowing the proper air transfer with the liquid in the tank. This reduces the mixing capability and
creates a low dissolved oxygen concentration which can cause the sludge to become septic. We have
contacted the manufacturer of the diffusers in hopes of finding an economical solution to the problem.
2.1 PRETREATMENT
During April, one industry was cited with a Notice of Violation (NOV) from the pretreatment
coordinator. Table 2.2 recognizes the industry who received the NOV and a description of the exceeded

discharge permit limits.

We will continue to work with the industries to ensure compliance with permit limits.
3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
Preventive maintenance was performed on all equipment as scheduled in April. There were 14
unscheduled maintenance tasks performed. All were minor except for the seeding and strawing
of the facility grounds to help improve appearance. A list of unscheduled maintenance work
orders is included as Attachment C.
Maintenance and repair expenditures for the month of April are detailed in Attachment D. Table
3.1 represents the total amount expended in April, and for the year. Table 3.2 includes the same
information for repair and replacement expenditures. Attachment E contains a detail of repair
andreplacement expenditures for the year.

4.0 FACILITY SAFETY & TRAINING
Safety inspections were conducted on April 10, 1995. The rating was 99%. The only deficiency
reported was that employees have not covered all phases of the facility's safety program. A copy
of the Safety Inspection report is included as AttachmentF.
On April 19 and April 26, 1995, Susan Tomiesello with the American Red Cross conducted First
Aid and CPR instruction for employees of the facility, and several personnel from various City
departments .
5.0 SEWER COLLECTION SYSTEM
During the month there were 11 sewer calls. Six of these calls were regarding sewer backups.
One of the backups was due to blockage of the City's main line. The remaining calls were due to
drainage problems in various locations of the City. Table 5.1, on the next page, shows the data
on the months sewer projects.
