1.0 EFFLUENT QUALITY
During September, effluent quality was within NPDES permit limits for CBOD
and TSS concentrations. Table 1.1 ~ummarizes the effluent quality data.
Attachment A contains Time Series Plots of daily CBOD and TSS values.

2.0 FACILITY OPERATIONS
Attachment B contains a list of septic haulers that discharged at the facility during
the month of September.
During September the treatment processes performed excellent. We did
experience high peak flows due to the heavy rains experienced throughout the
month. However, the high peak flows did not create any significant operational
problems.

The lawn was power grass seeded in an effort to improve the appearance of the
facility.
2.1 PRETREATMENT
[
During September, Wyandot Inc. received a Notice of Violation (NOV) for
exceeding their Oil & Grease (0&0) discharge permit limit. The excursion was
detected September 19, 1996. Wyandot also failed to submit a compliance report
that was requested by the pretreatment coordinator for violations that occurred'in
August. The report is to outline Wyandot's plan in eliminate O&G concentration
and pH discharge permit violations.
New permit limits were calculated and issued to Profile Extrusions. The new
limits have been issued due to the recent modifications of their treatment process.
Annual monitoring and sampling of Pfau & Sons Inc. was performed in
September. No deficiencies within their operation or violations of their discharge
permit were detected.
The monitoring frequency of IWR Inc. was reduced during September.
Monitoring by pretreatment personnel had been significantly increased last
January, due to improper operation and violations of their discharge permit. Since
January, IWR has had no significant problems within their treatment process or
discharge permit violations. Pretreatment personnel will continue to perform
routine inspections and random sampling of IWR's discharge to insure continued
compliance with their discharge permit limits.
We will continue to work with all industries, to ensure compliance with their

3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
D
Preventive maintenance was performed on all equipment as scheduled in
September. There were 21 unscheduled maintenance tasks performed. All were
minor repairs except for:
~ Cleaning and repair of the fine bubble air diffusers in Digester No.1.
~ No. 1 pump at the 10th street lift station was removed from service for
repair. The mechanical seal failed which caused severe damage to the
motor's electrical windings. Weare negotiating with the Manufacturer's
representative in order to recover part of this expense through warranty.
~ While performing routine preventive maintenance at the 10th street lift
station, a problem with the No.2 pump's wear ring and failure of the
mechanical seal was detected. The pump has been removed from service
in order to complete necessary repairs. We are negotiating with the
manufacturer's representative to recover part of this expense through
warranty.
~ Replaced the control valve to the No.2 barscreen compactor.
~ During routine inspection, personnel noticed excessive noise and vibration
coming from the No.1 pump at Riverport II lift station. The pump was
removed from service and a wooden 2x4 was found lodged in the pump's
impeller, thus causing damage to the power cord. The pump is being
repaired at this time.
~ Spring Street lift station No.2 pump drive shaft was repaired.
A list of unscheduled maintenance work orders is included as Attachment. B.
Maintenance and repair expenditures for the month of September are detailed in
Attachment C. Table 3.1 represents the total amount expended in September, and
for the year. Table 3.2 includes the same information for repair and replacement
expenditures. Attachment D contains a detail of repair and replacement
expenditures for the year.

Safety inspections were conducted on September 20, 1996. The rating was 95%.
The deficiencies reported were:

. All employees have not received first-aid and CPR training.

. Fire/Emergency evacuation is not posted.
A copy of the Safety Inspection report is included as Attachment F.
On September 18,1996 review of the Facility's Emergency Response plan was
conducted by the safety coordinator.

5.0 SEWER COLLECTION SYSTEM

During the month there were 45 sewer calls. The calls were related to the
following:
. Thirty-four reported sewer backup's
. Thirty-one were related to blockages within the residents line.
. Two were due to blockages within the City's main line.
. 1 was caused by roots.
. Four were due to odor complaints.
. Four were related to catch basin drainage.
. Three were caused by storm related backups
Table 5.1, on the next page, details the data on September's sewer projects.
