l 1.0 EFFLUENT QUALITY
During July, effluent quality was within NPDES permit limits for CBOD and TSS
concentrations. Table 1.1 summarizes the effluent quality data. Attachment A contains Time
Series Plots of daily CBOD and TSS values.

2.0 FACILITY OPERATIONS
A list of septic haulers that discharged at the facility during the month of July is at the
end of this report.
During July the treatment processes performed excellent. The mixed liquor suspended
solids (MLSS) concentration has been reduced in preparation of draining the oxidation
ditches for inspection. This should not have a significant impact on effluent quality.

JEFFERSONVILLE, IN
2.1 PRETREATMENT
During July, Wyandot Inc. received a Notice of Violation (NOV) for exceeding their Oil
& Grease (O&G) and pH discharge limits. The excursions were detected during
investigative sampling conducted from July 20, 1996 through July 31, 1996. Wyandot
has been asked to provide a report on how they plan to reduce O&G and pH
concentrations limits in their effluent.
We will continue to work with all industries, to ensure compliance with their industrial
discharge permit limits.
3.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
Preventive maintenance was performed on all equipment as scheduled in July. There
were 28 unscheduled maintenance tasks performed. All were minor repairs except for:
· Repair vactor truck hose.
. Repair belt filter press pumps.
. Replaced 6" gate valve on suction side at Camp Powers lift station.
. Repaired #2 pump & welded door hinges at Mill Creek lift station.
. Repaired #2 motor at Mill Creek lift station.
. Replaced alarm system probe at Mill Creek lift station.
. Repaired #1 motor & mounting pad at Sunset Mobile Horne Park lift station.
A list of unscheduled maintenance work orders is included as Attachment. B.
Maintenance and repair expenditures for the month of July are detailed in Attachment C.
Table 3.1 represents the total amount expended in July, and for the year. Table 3.2
includes the same information for repair and replacement expenditures. Attachment D
contains a detail of repair and replacement expenditures for the year.

4.0 FACILITY SAFETY & TRAINING
Safety inspections were conducted on July 24, 1996. The rating was 97%. The
deficiencies reported were:
~ Trip hazards were present at the final clarifiers and railings around the tanks were
not chained off.
~ All employees have not received first-aid and CPR training.
~ Guards to the lawnmowers were tied open.
A copy of the Safety Inspection report is included as Attachment F.
On July 24, 1996 review of proper chlorine and sulphur dioxide handling was conducted
by a representative of RVe Inc. at the facility.

5.0 SEWER COLLECTION SYSTEM
During the month there were 42 sewer calls. The calls were related to the following:
~ Twenty-five reported sewer backup's
~ Twenty calls were related to blockages within the residents line.
~ Five were due to blockages within the City's main line.
~ 1 was caused by roots.
~ Six were due to odor complaints.
~ Five were related to catch basin drainage
~ Five were caused by back ups
Table 5.1, on the next page, details the data on July's sewer projects.
