1.0 EFFLUENT QUALITY

During August, effluent quality was within NPDES permit limits for CBOD, TSS and NH-3.
There was an e-coli excursion on August 26th of 1,140 colonies/1 00 ml due to a mechanical failure.
The monthly average of 50.086 colonies/1 00 ml was still well within the permitted monthly limit
of 125 colonies/100 ml. IDEM has been notified and is aware of this event. Table 1.1
summarizes the effluent quality data. Attachment A contains Time Series Plots of Carbonaceous
Biochemical Oxygen Demand (CBOD) and Total Suspended Solids (TSS) values. Attachment B
contains Time Series Plots of Aeration Mixed Liquor Suspended Solids (MLSS) and Sludge
V olume Index (SVI).

3.0 FACILITY OPERATIONS
Attachment D contains a list of septic haulers that discharged at the facility during the month of
August.
During August, the treatment processes performed very well. All monitored parameters were well
witin range. Painting of#l, 3, and 4 clarifiers was completed.
3.1 PRETREATMENT
Pretreatment activities for the month included the following:
. Industries sampled during August included Cargo Clean and Pfau's.
. Inspections were made at Cargo Clean and Brinley-Hardy.

5.0 FACILITY SAFETY & TRAINING
A safety inspection was conducted on August 1, 2003. The rating was 100%. There were no
deficiencies reported. Our plant is still in excellent shape.
A copy of the Safety Inspection Report is included as Attachment G.
6.0 SEWER COLLECTION SYSTEM AND PREVENTATIVE MAINTENANCE

During the month, there were 29 sewer calls. Please see table 4.1, Monthly Sewer Call Report and
table 6.1, Monthly Collection Analysis Report, for a more detailed breakdown of monthly sewer
maintenance.

In the month of August trouble spots were cleaned. Collection crews televised to locate five taps.
Crews also vaced and cleaned lines and catchbasins as needed on the Tenth Street project. Three
manholes were cleaned on Brisco and turned over to the city for repairs. 178 catchbasin tops were
cleaned during rain events. Crews were at Twelfth and Locust televising for feeders in an 8" main
and it was also turned over to the city for repairs. Middle School, Crums Lane 2, Eastbrook,
Colonial Park, Creekstone, Utica 1 and 2 wet wells were cleaned for grease and solids on the
preventative maintenance program. All CSO gates were taken out and sent for repairs and
reinstalled with the addition of grease fittings and stainless steel stems to improve longevity and
better operation of gates. Stop floats were changed at Crums Lane 3, Utica 2, Louise Street and
Camp Powers on preventative maintenance program. Spring Street lift station pump #1 was rebuilt
and new u-joints and yolk were installed in the drive shaft. Crums Lane 1, pump #2 was taken for
winding repairs and reinstalled. Powerhouse pump #1 wear ring and impeller were repaired and
were put back into service. Cedarview lift station pod was cleaned for preventative maintenance.
Transducers and stilling well were cleaned at Tenth Street and Spring Street lift stations for
preventative maintenance.

Sometime early on the morning of the 26th, the WWTP v.;ater pumps became partially stopped up.
Pressure never got low enough to trigger the low water pressure alarm. During rounds, at
approximately 7:30am on the 26th, the process water pumps were operating with low chlorine
feed which is normal for that time of the morning.
As the WWTP flow increased over the next several hours, the automatic system called for an
increase in the chlorine feed rate. . Due to the low pressure from the plant water pumps, the
chlorinators were not able to feed the sufficient level of chlorine to the tanks. When the E-Coli
sa.mple was taken, at approximately 9:00am, therewas not enough chlorine being fed to assure
adequate disinfection. During the early afternoon rounds, the operator noticed that the chlorine
rate was unusually low. Upon investigation, it was found that the chlorinators were experiencing
low water pressure. Atthat time, the chlorinators were switched over to city water feed.
The process water pumps \vere taken apart, where pieces of a canvas tarpaulin were removed
from the pumps. The pumpswere flushed, and put back on line. The e-coli sample was taken
before the water supply was switched over to city water, which is the cause for the excursion.
I would like to point out that the e-coli count on the previous day was liS and the following day
was 125.

RE: E-Coli excursion of August 26, 2003
To Whom It May Concern:
Sometime early on the morning of the 26t\ the plant water pumps became partially stopped up.
Pressure never got low enough to trigger the low water pressure alarm. During rounds, at
approximately 7:30am on the 26th, the plant water pumps were operating with low chlorine feed
which is normal for that time of the morning.
As the plant flow increased over the next several hours, the automatic system called for an
increase in the chlorine feed rate. Dueto the low pressure from the plant water pumps, the
chlorinators were not able to feed the sufficient level of chlorine to the tanks. When the E-Coli
sample was taken, at approxiillately 9:00am, there was not enough chlorine being fed to assure
adequate disinfection. During the early afternoon rounds, the operator noticed that the chlorine
rate \vas unusually low. Upon investigation, it was found that the chlorinators were experiencing
low water pressure. At that time, the chlorinators were switched over to city water feed.
The plant vvater pumps were taken apart, where pieces of a canvas tarpaulin were removed from
the pumps. The pumps were flushed, and put back online. The e-coli sample was taken before
the water supply was switched over to city water, which is the cause for the excursion.
I \vould like to point out that the e-coli count on the previous day was 115 and the following day
was 125.
