t. Following are summaries for operation and maintenance at the wastewater treatment plant, and maintenance of the collection
system and lift stations for the month of October, 2006

).>- Effluent quality was within NPDES permit limits.
).>- Removal percentages for the month, BOD 98.3%, TSS 97.2%, and NH3 99.5%.
).>- Settleable solids averaged 350 mg/I for the month, settlleablability is very good at this time.
).>- There were 16 wet days (defined as a day having at least 0.1 inch of rainfall and three days afterward) resulting in an
average plant flow of 8.58 MGD, and 15 dry days with an average flow of 5.11 MGD. The flow for the plant was 136%
).>- Ofthe design flow.
).>- The facility received an average influent BOD loading of 15741.7lbs/day, (Design- 10581 Ibs/day)
).>- The facility received an average influent TSS of 14309.2 Ibs/day; (Design - 10 I 05 Ibs/day)
).>- The facility received an average influent Ammonia of 739.4 Ibs/day, (Design-I 30 I Ibs/day)
).>- The maximum rainfall event during the month was on October 16 at 2.15".
).>- Plant is operating well, all equipment is online.

. Crews cleaned 8,135 ft. of sanitary sewers. (YTD 54865)
. Crews cleaned 7,274 ft of Storm I CSO sewers. (YTD 46421)
. Crews televised 3, 155 ft. of sewer Lift station and collection system
. lines. (YTD 11397)
. All 23 trouble spots were cleaned and or checked during the month.
. Crews cleaned 6 C/B and 15 C/B tops off.
In the month of October crews made 29 sewer calls 27 being residents or other problems and 2 being city's lines.
. 1503 Varble Ct. crews cleaned 455' during a several day period but line was. qbstrllcted by roots,
city dug and laid new line.

. 1205 Redwood Dr. crew cleaned and vactored 250' to open line, crew had hard time with M/H lid,
digging crew put in new ring.
. 119 E. Maple St. crew cleaned 100' to try and help resident they would need a plumber.
. 419 Chippewa Dr. crews cleaned 400' to try and help resident they would need a plumber.
Collection crews worked with different departments of the city on various projects and a few businesses jobs
around town.
. Crews cleaned and vactored I C/B and 185' of storm line in America Place for Jim S.
. Crew took Vactor truck to Crums Lane to vactor out ditch for digging crew they were putting in
line and a cia for resident.
. Crews worked several days on Varble Ct. the city's installing new line.
. Crews tried to clean and televise storm line on Ewing Lane but line was collapsed for Mike L.
. Crew located sewers on Francis Ave for Jerry C.
. Crews worked with JTL crew at the old plant locating lines.
. Crews located storm line on 12th St. for Jim S.
. Crews cleaned 175' and televised 175' on Wall St. and Maple St. to locate taps for Mike L.
. Crews cleaned 219' and televised 133' to locate tap at 719 Ohio Ave so digging crew could install
a cia at road for Jim S.
. Crews televised 1,090' to check line at Veterans Pkwy for JTL crew.
. Crews televised 390' on Sundancer Dr. to locate tap for Jerry C.
. Crews televised 100' at lot # 19 Rose Lawn Dr. to locate tap for Jerry C.
. Crews cleaned and vactored 200' of storm line at Bishop and Fallow Dr. for Jim S.
Collection crews street pre-maintenance schedule
. Crews cleaned and vactored 2,360' of the Market St .interceptor line.
. Crews televised 1,0 IT to locate 5 taps for different residents.
. Crews televised 250' to see what caused back up at 1205 Redwood Dr. line was full of roots
crew used root cutter to remove them.
. Crews cleaned and vactored 1,240' in different areas of the city.
. Crews cleaned and vactored 2,030' on Kewanna Dr from 8th St to Market St. as preventative
cleaning.
. Crews cleaned and vactored 1,450' on Alvin Ct. and Lindy Dr. in the Landsburg Cove subdivision
as preventative cleaning.
. Trouble spot list was completed 6,345' of sewer lines cleaned and vactored.
. A crew vactored out 2 wet wells due to build up of grease and solids.
. All CSO gates were greased and exercised.
. Crews cleaned the tops of 15 C/B during the rainy weather.
. Crews cleaned and vactored 2 M/H and 6 C/B around town.
Collection crews station pre-maintenance schedule
. All lift stations floats, switches, pumps, alarms, electrical and mechanical components were
cleaned, exercised, and inspected for proper operations.
. Generators at 10th St, Mill Creek and Spring St. were operated, inspected and exercised for
proper operations.
. Delta electric was called to replace a stop float at Pleasant Run station.

. Crews opened Check valve on pump # I at Power House station and removed a piece of wood
put back on line.
. Crews cleaned rags off of floats at Riverport #2
. Crews pulled pump # I at Crums Lane #2 removed rags and put back on line.
. Crews pulled pump #2 out of Utica #2 for repairs.
. Crews put signs on new stations.
. A crew vactored out air release valve boxes for Riverport #2 force main.
. Crews back flushed both pumps at Cherry Creek the floats were tangled.
. Crews opened check valve on pump #2 at Arctic Springs to check for debris.
. Bar screens were cleaned as needed.
Repairs made in the collection system
. Delta electric installed new parts to Crums Lane # I electric panel.
. Delta electric installed new parts to Silver Creek electric panel.
. Delta electric fixed lighting in bar screen room at 10th St. station.
. Delta electric installed rebuilt soft start on pump #3 at 10th St station.
. Repaired 2 hydraulic cylinders for vactor trucks.
. Took TV van to Mt Sterling to have cameras repaired.
. Took sludge truck to vehicle maintenance for repairs.
. Took dodge pickup to vehicle maintenance for front end repair.

3.0 FACILITY OPERATIONS

Jeffersonville Wastewater Treatment Facility
Monthly Operations Report
During October, the treatment processes performed very well. All parameters were will within
IDEM limits. Average influent flow was 136% of plant design capacity. Removal rates for the
month were BOD removal- 98.3%, TSS removal- 97.2%, Ammonia removal- 99.5%.
3.1 PRETREATMENT
Pretreatment activities for the month included the following:
. New environmental coordinator was contracted at Altec, Inc.
. Quarterly industrial monitoring reports were submitted to IDEM.
4.0 SEWER MAINTENANCE CALLS
Table 4.1 represents all sewer maintenance calls for the month.

The new safety coordinator has not been named as of the date of this report.
Simplex-Grinell inspected all ofthe fire equipment on plant as well as all lift stations in the
