Monthly Operations Report
1.0 EFFLUENT QUALITY
During August, effluent quality was within NPDES permit limits for CBOD, TSS and NH-3. Table 1.1
summarizes the effluent quality data. Attachment A contains Time Series Plots of daily Carbonaceous
Biochemical Oxygen Demand (CBOD) and Total Suspended Solids (TSS) values. Attachment B contains Time
Series Plots of Aeration Mixed Liquor Suspended Solids (MLSS) and Sludge Volume Index (SVI).

3.0 FACILITY OPERATIONS

Attachment D contains a list of septic haulers that discharged at the facility during the month of August.

During August the treatment processes performed very well. The facility experienced normal rainfall for the
month.
The sludge settleability and Sludge Volume Indexes (SVIs) in the secondary treatment process were normal for
the month of August.
Smith Contractors has been awarded the Influent Flume and Clarifier project. They are on site and
proceeding. This project is nearing completion. The clarifier should be completed near the end of October.

Pretreatment activities for the month of August include:
. Voss Clark was determined to be out of compliance on 8-30-00 when the Jeffersonville Wastewater
n Laboratory staff performed their annual sampling. Voss Clark had a zinc result of 4.64 mg/L. This
result exceeds the permitted limit. Voss Clark was issued a Notice of Violation (NOV).
. Annual inspections were performed at Dallas, Voss Clark, Cargo Clean, and Alumnitec.
. Annual sampling was performed at Voss Clark and Dallas.
. Oil and Grease was found entering the collections system on the morning of
August 14, 2000 at the Silver Creek Business Center. The lift station and surrounding manholes
were inspected and samples were collected. The only industry that had a similar discharge was Cargo
Clean. Holly Vail, the Environmental Manager for Cargo Clean, was notified of the problem the lift
station was experiencing.
. There was a meeting held with representatives from the City of Jeffersonville and Cargo Clean
discussing issues, such as, the metals violations and getting permission to install a pretreatment
system. Cargo Clean will be issued a consent order until the pretreatment system is installed.
4.0 PREVENTIVE AND UNSCHEDULED MAINTENANCE
Preventive maintenance was performed on all equipment as scheduled for August. There were 15
unscheduled maintenance tasks performed.

A list of unscheduled maintenance work orders and sewer calls is included as Attachment E.
Maintenance & Repair expenditures for the month of August are detailed in Attachment F . Table 4.1
presents the amount expended in August. Table 4.2 includes the same information for Repair &
Replacement expenditures. Attachment G contains detail of Repair & Replacement expenditures for August.

5.0 FACILITY SAFETY & TRAINING

A safety inspection was conducted on August 15, 1999. The rating was 100%. There were no deficiencies
reported. Our plant is in great shape.
A copy of the Safety Inspection report is included as Attachment I.
SfFety training was provided by our Safety Coordinator on Machine and Hand Tool Safety for the month of August.

\:ollection system personnel have been cleaning and preforming preventive maintenance work at various liftstations.
Several stations have been painted inside and out. Catch basins have been checked and cleaned as needed. Four of the
CSO gates have been repaired and are being scheduled to be put back in place. During inspection of various lift
stations there were several maintenance items found that need to be addressed. Check valves and other routine
rhaintenance items have been placed on the maintenance schedule to be completed in the following months. After
r~pairs are completed, the lift stations will be scheduled for painting. The lift stations scheduled for painting are as
follows: lO'h Street, Golfview, Silver Creek Business Center, Cherry Creek, Eastbrook, Arctic Springs, Crums Lane
III, Mill Creek, Camp Powers, Powerhouse, and Eastern Boulevard.
